As an NP, here’s why “eat less, move more” doesn’t work for so many women. 👀

It sounds simple. Calories in, calories out. And for some people, it genuinely is. But for a huge number of women doing exactly that and still not losing weight, it’s not a mystery — it’s biology nobody’s ever explained to them.

Chronic undereating doesn’t speed up fat loss. It slows your metabolism down, because your body adapts to survive on less instead of releasing weight faster.

Elevated cortisol — from stress, including the stress of restricting food — keeps your body in fat-storage mode, especially around the belly, no matter how clean you’re eating.

Insulin resistance can sit there for years before it ever shows up on standard bloodwork, quietly making it harder for your body to access stored fat for fuel.

And hormonal shifts in thyroid, estrogen, and progesterone change how and where your body holds onto weight, especially with age. None of that responds to eating less.

This isn’t about trying harder. It’s about understanding what your body actually needs.

You’ve been lied to about what “normal” bloodwork actually means. 🧬

National health data shows that among people with completely normal blood sugar, roughly 1 in 3 had high insulin resistance when tested further. That’s not a rare exception — that’s a massive number of people walking around with a hidden driver of fatigue, cravings, and stubborn weight, while their labs say everything is fine.

Here’s why this happens: fasting glucose is one of the last markers to change. Insulin rises first, often for years, working overtime to keep your blood sugar looking normal. By the time glucose actually moves out of range, insulin resistance has typically been building for a while already.

This is exactly why “your bloodwork is normal” isn’t the same as “nothing is wrong.” Fasting insulin and HOMA-IR tell a completely different story than glucose alone — and most standard panels never check them.

You could be a postmenopausal woman under 5’2, holding weight around your gut, unable to sleep unless you’ve got five fans running and the AC down to the 60s — and everyone around you is still saying “just exercise harder and eat less.” 🧬

Let’s be honest about what’s actually happening here. A shorter frame means a smaller baseline calorie need, so “eat less” often isn’t even realistic without real consequences to your metabolism. Weight settling around the belly after menopause is a hormonal shift, not a discipline problem. And needing the room ice cold just to sleep? That’s your body running its own internal heatwave — a hallmark of the hormonal changes happening underneath, not something more fans can actually fix.

None of this responds to “try harder.” It responds to actually understanding what’s going on with your metabolism, your hormones, and your sleep — together, not separately.

I hear this more and more: “I’m on the medication, doing everything right, and I’ve barely lost anything” — or it worked for a while, then just stopped.

Here’s what I actually look at when that happens:

Thyroid conversion — not just TSH. Cortisol patterns — a single blood draw doesn’t capture the full picture. Insulin resistance — it can blunt how well a GLP-1 works, even at the right dose. Gut inflammation — affects nutrient absorption and hormone metabolism. And whether real dietary changes have actually been made, since the medication reduces appetite but food quality still matters.

GLP-1s are a genuinely great bridge tool. I prescribe them myself. But they were never meant to work in a vacuum — and that part rarely gets explained.

Early in my career, that’s genuinely what I believed. Calories in, calories out. If the weight wasn’t moving, she just wasn’t trying hard enough.

I was wrong.

Postmenopausal weight gain isn’t a math problem — it’s a hormone problem. Estrogen decline changes where and how your body stores fat. Cortisol becomes harder to regulate. Insulin sensitivity shifts. None of that responds to “eat less, move more,” and telling a woman it does just leaves her blaming herself for something her body is doing on its own.

I don’t practice that way anymore. Now I actually test — hormones, thyroid, cortisol, gut — because “try harder” was never the answer. It was just the easiest thing to say.

Most bloodwork checks one marker — TSH — and calls it a day. But TSH only tells you what your pituitary is signaling. It doesn’t tell you what your thyroid is actually producing, what your body can use, or whether stress is quietly blocking conversion.

A full thyroid panel looks at Free T4, Free T3, Reverse T3, and thyroid antibodies — together, not in isolation. That’s often where the real answers are hiding.
