NEW INSIGHTS INTO THE STATIN-CHOLESTEROL CONTROVERSY

This is a review article I wrote in the Journal of American Physicians and Surgeons (JPANDS) discussing the issues with statin medications and the importance of higher levels of cholesterol for optimal health and well-being.

🧠 Cholesterol is a neuroprotective nutrient that the body needs in sufficient quantities (160+ mg/dL) to function optimally. Higher levels of cholesterol protect the brain and nervous system against a variety of chemicals and toxins due to its highly lipophilic nature.

⚗️In chemistry and pharmacology, “lipophilic” means “fat-loving” and based on the principle of “like dissolves like” - cholesterol attracts and binds to organic toxins - preventing them from damaging your body’s cells.

🧠 Many organic toxins are highly lipophilic meaning they’re attracted to and easily dissolve in fat. Since the brain is roughly 60% fat - organic toxins are attracted to and can accumulate in the brain and nervous system, which is why we need higher cholesterol levels, now more than ever.

Your blood and your neurons hold very different amounts of vitamin C.

Vitamin C cannot cross the blood-brain barrier on its own. The barrier is sealed tight and has no transporter for it. So the body pumps it in.

A protein called SVCT2 does the pumping, in two steps. First it carries vitamin C from blood into the cerebrospinal fluid, raising the level about 4 times. Then a second SVCT2 pump, sitting on the neuron itself, pulls it inside again. The neuron ends up holding roughly 200 times what is in the blood.
The cells that surround neurons, the glia, mostly do not have the pump. They sit in the same fluid, bathed in the same vitamin C, and hold only a fraction of what the neuron holds.

A neuron spends sodium and energy to do this against a steep gradient. It pays that cost for three reasons. It runs about 10 times the oxidative load of the cells around it, and vitamin C mops up the wreckage. It uses vitamin C as its single biggest antioxidant supply, bigger than glutathione, and uses it to recharge vitamin E. And it needs vitamin C as a one-electron donor to run the enzymes that build norepinephrine, collagen, and myelin.

The brain protects this pool harder than almost any tissue in the body. In scurvy, the liver runs dry while the brain still holds about a quarter of its normal vitamin C. The flip side is the part the marketing skips: you cannot flood it. Oral vitamin C raises brain levels by about 20 percent at most. The brain decides how much it keeps, not the bottle.

Magnesium gets sold as the thing that "activates" your vitamin D. The real story is more precise, and worth getting right.

Vitamin D, whether from sun or food, is inactive when it first enters your body. Two enzymes have to switch it on. The first lives in the liver and adds one chemical group, turning vitamin D into 25(OH)D, the storage form your blood test measures. The second lives in the kidney and adds a second group, producing the active hormone your cells actually use.

Both of those enzymes are cytochrome P450s. The metal doing the chemistry at their core is iron, held in a heme group. Iron is what binds oxygen and inserts the modification onto the vitamin D molecule. So if you want to name the metal that powers vitamin D activation, it is iron, not magnesium.

Magnesium is not that metal. It is not sitting in the active site doing the catalysis. What it does is support the wider system: the surrounding enzymatic machinery and the hormone signaling that drives this pathway. That is a real role, but it is a supporting one, and the popular "magnesium is the cofactor that activates vitamin D" line blurs the distinction.

What makes magnesium worth talking about is what shows up in the blood. In a randomized controlled trial of 180 adults, magnesium supplementation did not simply raise vitamin D across the board. It depended on where people started. When baseline vitamin D was low, magnesium raised it toward the optimal range. When baseline was already high, magnesium pulled it back down toward that same range. It behaved like a tuner, not an accelerator. The authors concluded that good magnesium status helps optimize vitamin D status.

That is the useful takeaway. Vitamin D activation is a multi-part system. The catalytic engine runs on iron. Magnesium is one of the supporting inputs that helps the whole thing settle where it should. Chasing your vitamin D number alone, while ignoring the rest of the machine, misses how the system actually works.

One honest caveat. The tuning effect comes from a single trial of 180 people. The mechanism is sound and the result is striking, but it has not been replicated at scale, and the broader claim that magnesium status shapes vitamin D should be read as supported, not settled.
