Home Health Vs the Hospital. Which is better?

It’s more of a question of pace Vs better.

In the hospital, I had 2min to read a chart and 7 other patients waiting. Good care happened. Not great care.

In a home, I have an hour. I can ask how the kids are. I can notice the family photo on the wall.

I went to nursing school because I wanted to take care of people. Not patients. People.

Home health gave me back the part of nursing I went to school for.

A hospital discharge med list is its own language. Let me translate.

→ NEW = highest risk. New medication, never been on it before. Confirm with home health.

→ CHANGED = the dose moved. Did anyone tell the pharmacy?

→ DISCONTINUED = should not be in the pillbox. Pull those bottles aside.

→ Brand name? Ask for generic.

→ Shorthand like q.d. or b.i.d.? Ask the pharmacy to write it in plain English.

Reading the list is half the battle. Don't leave the hospital without doing both halves.

5 things to have ready before the home health nurse arrives.

1. The medication list (every bottle, every dose, every vitamin)

2. The hospital discharge papers, the whole stack

3. The insurance card

4. A list of questions you've been afraid to ask anyone else

5. Anyone in the family who helps with care decisions

If you have those five ready, the first visit is more useful in 60 minutes than three visits without them.
