We are a 501(c)(3) agency contracted with CSOC to serve youth aged 5-21 experiencing behavioral, emotional, social, developmental, and mental health challenges. The Tri County Care Management Organization takes a family focused and community-based leadership role in supporting the needs of youth who are experiencing behavioral, emotional, social, developmental, and mental health challenges.

On Friday, staff from all 15 of New Jersey’s Care Management Organizations (CMOs) joined DCF leadership, including the Commissioner and Deputy Commissioner, formerly served youth and families, system partners and state legislators to celebrate 25 years of service and impact.

For a quarter century, CMOs have helped New Jersey’s youth and families navigate mental health, behavioral health, substance use, and intellectual and developmental challenges through individualized, community-based wraparound care coordination.

Here's to 25 years of strengthening families, building partnerships, and helping youth thrive in their homes, schools, and communities!
