Lamn Care provides housing, hospitality services and personal care services for adults who can live independently and make decisions on their own behalf but require a supportive environment due to physical and functional health challenges. Whether our clients prefer to receive care in the comforts of their home or in a structured living arrangement, we have a team of professionals who can provide them with the care that they deserve. SERVICES PROVIDED
24 Hour Emergency Services Assistance: 24-Hour Emergency Assistance for people on a Disability Waiver is on-call counseling and problem solving and/or immediate response for help at a waiver recipient's home due to a health or personal emergency. Adult Companion Services: Adult Companion Services are non-medical social support services for people with disabilities or older adults who need supervision, care or assistance. Homemaking: help with general household activities when a person is unable to manage the home, or when the person regularly responsible for these activities is temporarily abse. Night supervision: Night Supervision is overnight assistance and monitoring in the home of a person for a period of no more than twelve (12) hours in a 24-hour period or unable to manage the home. Personal care Assistance: Activities of Daily Living: eating, toileting, grooming, dressing, bathing, transferring (such as getting in and out of bed), mobility, and positioning
* Health-related functions, which, under state law, can be delegated or assigned by a licensed health care professional to be performed by a PCA
* Instrumental Activities of Daily Living: meal planning and preparation, managing finances, shopping for essential items, performing essential household chores, communication by telephone and other media, and participating in the community
* Redirection and intervention for behavior including observation and monitoring. Personal Supports: The service is provided in the home of the person with a developmental disability or in the community to achieve increased independence. Respite ( in home): Respite provides care on a short-term basis for a person who cannot take care of themselves, and who is usually taken care of by an unpaid family member or friend. The program is designed to provide temporary coverage for the unpaid caregiver during an absence or need for relief. Transportation: Rides to access services, community activities, and resources for people on a waiver or Alternative Care (AC) program, when other transportation is not available. Independent Living Skills (ILS): Training services develop, maintain, and improve a person's community living skills in the areas of:
* Communication
* Community living and mobility
* Interpersonal
* Reduction/elimination of maladaptive behavior
* Self-care
* Sensory/motor development involved in acquiring functional skills
Individualized Home Supports: Individualized Home Supports uses a person-centered approach to support what is important to and for a person. The service combines both training and supports into a single, flexible service. Training is provided in the person's home and community, with the goal to develop, maintain or improve the person's skills related to activities of daily living. Training is available to the person on the waiver or the person's family, including extended family.
