Hi everyone! I wanted to take a minute to re-introduce myself for those who may be new here.
I’m Sarah, the occupational therapist behind HomeBridge Network. I’ve been an OT for 9 years, working with individuals and families in hospital and inpatient rehab settings to help prepare them for the transition home.
Over the years, I’ve seen how overwhelming it can feel for families when a loved one is coming home after an illness, injury, or hospitalization. There are so many questions: How do we set up the house? How do we help them safely? What do we do when something changes? How do we know we’re doing the right thing?
I created HomeBridge Network to help answer those questions and provide the education, guidance, and support families need, before and after discharge.
Inside HomeBridge Network, you’ll find:
• 70+ instructional videos on preparing for discharge, home setup and modifications, daily care needs, transfers, assistive devices, and supporting cognition at home
• A community of other family caregivers who understand the challenges you’re facing
• The option for personalized 1:1 or group virtual support with me, an occupational therapist
My goal is to help families feel more prepared, confident, and supported as they care for their loved ones at home. Whether you’re preparing for a loved one to come home, they’ve recently returned home, or you simply want to be better prepared, HomeBridge Network was created for you!
