Changes are coming to the Support at Home End-of-Life Pathway in March 2027.

From 8 March 2027, eligible participants who remain living at home after their initial 12 weeks will automatically receive a further 12 weeks of support and approximately $25,000 in additional funding, without needing reassessment.

To help aged care providers prepare, the Australian Government Department of Health, Disability and Ageing is hosting free information sessions covering the changes, with opportunities to ask questions.
