ANNUAL REVIEWS

Long term health conditions require regular reviews to help you stay well.

During your Appointments relevant tests may be requested and your condition reviewed. Medication will be checked and any concerns can be discussed to ensure your getting the right care and support.

If you receive an invitation, please book your appointment as soon as possible.

Changes can be identified early with regular reviews and keep you as healthy as possible.

Your GP practice receives important information from hospitals, including discharge summaries, outpatient letters and test results, but GP practices and hospitals often use different computer systems.

Because of this, GPs may not be able to see:
📄 Every hospital note
📄 Scanned documents
📄 Internal comments or records
📄 Real-time updates from hospital teams

Many records are shared electronically, but this process depends on the hospital system and is not always immediate.

If you have a question about a hospital appointment, test, treatment or hospital letter, the hospital team is usually best placed to help directly.

Crags Health Care – Appointments

At Crags Health Care, we offer a wide range of ways to help you access NHS services.

For minor health concerns, we encourage patients to seek advice and treatment from their local pharmacy.

You can also get help with symptoms via NHS 111 online: https://111.nhs.uk/

We offer pre-bookable appointments, which can be arranged up to four weeks in advance.

For urgent issues, a limited number of on-the-day appointments are available from 8:00am, subject to availability.

We also provide Saturday appointments for selected conditions. These must be booked in advance and are held at Castle Street Medical Centre, Bolsover.

Physiotherapy (Musculoskeletal Problems)

Appointments with our First Contact Physiotherapist are available:

>Thursdays at Whitwell Surgery

>Additional days may be available at other sites

>Virtual (Zoom) appointments can be offered. where appropriate

First contact Physio

The First Contact Physiotherapy (FCP) service allows patients to see a physiotherapist directly for musculoskeletal issues without needing a GP referral, providing quicker access to assessment and treatment.

Overview of First Contact Physiotherapy

The First Contact Physiotherapy service is designed to provide patients with immediate access to physiotherapy for musculoskeletal (MSK) conditions, such as back pain, joint pain, and other related issues. This service is available in many GP practices across the NHS, allowing patients to consult with a physiotherapist directly, which can lead to faster diagnosis and treat

Crags healthcare offer face 2 face appointments every Thursday at the Whitwell Health centre. Alongside these appointments there are some availabilities at other clinics on different days if you can travel.

Good afternoon,
We are currently receiving a lot of enquiries with regards to hearing aid batteries .

Unfortunately the hospitals have stopped sending these to community surgeries.

The process has now changed and you need to contact your hearing provider to get these.
