Healthcare costs keep rising—and employers feel it in renewals, stop-loss rates, and unpredictable high-dollar claims. One of the biggest cost accelerators isn’t the surgery itself—it’s the complicated surgical episode: a preventable ICU admission, prolonged length of stay, or an avoidable readmission.

Healthcare costs are rising fast, and across employer-sponsored plans, a large share of that increase is driven by musculoskeletal (MSK) conditions—especially spine surgery (including fusion) and major joint replacement.
MSK conditions represent a major portion of employer medical costs.
That matters because the highest-cost MSK episodes tend to cluster around a small number of procedures, including:
• Spinal fusion / complex spine surgery
• Total joint replacement (knee, hip, shoulder—especially revisions)
These procedures can be life-charging, but they can also be highly variable in necessity, approach, and total cost, depending on provider, site of care, and patient selection.
Employers and patients are not just paying for “needed care.”
A recent analysis by the Lown Institute (using Medicare and Medicare Advantage claims) found that U.S. hospitals performed more than 200,000 unnecessary back surgeries over three years, costing over $1.9 billion.
In a peer- review of second-opinion services in spine surgery it was found that second opinions often recommend less invasive or non-surgical approaches compared to first opinions and may reduce surgery rates in the short term.
