Title of Study: Borderline Hip Dysplasia - Best Treated with Hip Arthroscopy or Periacetabular Osteotomy?

This study reviews literature comparing isolated hip arthroscopy, periacetabular osteotomy (PAO), and combined procedures for borderline hip dysplasia (lateral center-edge angle 18°–25°).

Borderline dysplasia represents a treatment "gray zone" because symptoms may arise from instability, femoroacetabular impingement, labral tears, or mixed pathology.

Both arthroscopy and PAO demonstrate significant improvements in patient-reported outcomes with low mid-term conversion rates to total hip arthroplasty.

No single procedure is superior for all patients; outcomes depend on appropriate patient selection and surgical technique.

Comprehensive clinical evaluation, imaging, and identification of the primary pain generator should guide individualized treatment decisions, while further long-term comparative studies are needed.

We’d like to recognise several long-standing members whose dedication to ISHA and to advancing excellence in hip preservation has spanned many years...

Dr. Soshi Uchida MD, PhD., has been practicing his passion, orthopaedic surgery, for the last decade. His principles of sports medicine emphasize a comprehensive team approach in the conservative treatment of injuries with the goal of helping the patient return to a healthy state, focusing on the prevention of new or recurrent injuries.

When surgery is necessary, Dr. Uchida's specialty in arthroscopic techniques, especially hip arthroscopy, emphasizes less invasive surgery for a potentially easier and quicker recovery. He is a well-known figure in hip arthroscopic surgery, lecturing, travelling and teaching widely.

Dr Uchida has been a highly active member of ISHA for many years, most recently serving as a General Board member and Host Chair for last year’s wonderful Annual Scientific Meeting in Fukuoka.

Title of Study: The conversion rate to total hip arthroplasty after hip arthroscopy in patients with femoroacetabular impingement syndrome is 8% and is affected by older age, cam morphology and osteoarthritis

Registry study (Danish Hip Arthroscopy Registry)

Purpose: to investigate the proportion of THA conversion in a large national Danish cohort of patients with FAIS who had previously undergone hip arthroscopy.

Parameters: HAGOS, HOOS, Hip Specific Sport Scale, PASS. Survival analysis : Kaplan-Meier was performed.

A total of 5109 patients were eligible and invited to the survey. Of which, 2383 patients (46.6%) responded. Among them, 406 patients had received THA at a median of 4.2 years after hip arthroscopy.

Among patients treated with THA, 56% participated in sports activities and 62% reported an acceptable symptom state.

Conclusions
A low number of conversions to THA (8%) was observed. Older age decreased lateral joint space width, Tönnis grade > 0, higher alpha angle and femoral cartilage pathology were associated with conversion to THA.

Title of Study: Return to Sport in Professional Athletes With Borderline Hip Dysplasia After Hip Arthroscopy for Femoroacetabular Impingement

Cohort study; Single centre

Purpose: To assess RTS rates exclusively in professional athletes with borderline dysplasia after hip arthroscopy for treatment of FAI.

40 professional and Olympic athletes (42 hips) with borderline dysplasia (lateral center edge angle [LCEA] 18°-25°) underwent hip arthroscopy for FAI

Patients undergoing primary hip arthroscopy demonstrated a 79% RTS rate, whereas patients undergoing revision hip arthroscopy demonstrated a 25% RTS rate

Among athletes who successfully returned, RTS occurred at a median of 7.5 months (range, 3.1-24.3 months) after surgery

Older age (P = .041) and history of previous hip arthroscopy (P = .008) were associated with a lower likelihood of return to professional sport.

Conclusion
In the primary setting, professional athletes with borderline hip dysplasia who underwent hip arthroscopy for FAI demonstrated a 79% RTS rate.

Title of Study: Excessive Femoral Anteversion Associated With Worse Clinical Outcomes Following Hip Arthroscopic Surgery and Higher Prevalence of Anteroinferior Labral Tears in Patients With Femoroacetabular Impingement Syndrome

Retrospective cohort study

Purpose: To compare intraoperative findings associated with instability and clinical outcomes after hip arthroscopic FAI surgery in patients with normal anteversion (NA) and excessive femoral anteversion (EA).

34 hips in NA group (femoral anteversion 10°-25°) and 16 hips in EA group (femoral anteversion > 25°)

Parameters: Clinical outcomes measure (iHOT-12, HOS-ADL, HOS-SP, mHHS), PASS, MCID and intraoperative findings suggestive of instability.

Anteroinferior labral tears were significantly more prevalent in the EA group (75% vs 29.4%; P = .0051)

Postoperative PROMs at 2 years were significantly worse in the EA group.

Three reoperations occurred in patients with EA, with femoral anteversion angles of 29°, 32.5°, and 33.5°

Conclusions: Patients with EA had inferior outcomes of hip arthroscopy and a higher prevalence of anteroinferior labral tears.
