ATTENTION: ALL INCOMING FIRST-YEAR STUDENTS

Please be informed that your medical examinations at Libertad Polyclinic will be held from Monday to Thursday (July 27–30,2026), every afternoon. The process will be on a first-come, first-served basis. Please observe the proper dress code at all times. Thank you.

Important Notes:

- For students who availed of the laboratory package last week: Your results are already available at the polyclinic. All you need to bring is the medical form provided by the school.

- For students who wish to take their medical examinations (and have already completed their own labs): Please bring all your laboratory results along with the school-provided medical form.
-For students who have already completed their medical examination:
Please upload your accomplished school-provided medical form using your personal UALC account.
