People often think surgeons solve every problem by operating.

Today I reviewed a patient with a small wound that hadn’t completely healed. I could have taken them back to theatre and stitched it again.

But after examining it carefully, I decided the best treatment was to let the body continue healing, with good wound care and close follow-up.

Surgery isn’t just about knowing how to operate. It’s about knowing when not to.

Good judgment is just as important as good technique.

Sometimes the best surgical decision isn’t to create another wound.

When a wound has a small, clean, superficial separation with no infection or tension, it’s often better to allow it to heal by secondary intention (secondary granulation) rather than refreshing the wound edges and suturing it again.

Good surgery isn’t just about technical skill—it’s about choosing the option that gives the patient the best chance of healing.

Every wound is different, which is why treatment should always be individualised.
