A missed period is not the end of the story — it is the first clue.

At first, everyone thinks of pregnancy. And clinically, that is exactly where the investigation should begin: rule it out first. But once pregnancy is excluded, the story becomes more interesting. The missing period may be coming from the ovaries, the brain, the thyroid, the pituitary gland, the uterus, medication effects, or even the body’s response to stress and low energy availability.

Think of the menstrual cycle like a conversation between the hypothalamus, pituitary gland, ovaries, and uterus. If one part of that chain goes silent, the bleeding pattern can change or stop. PCOS may show irregular cycles with acne or hirsutism. High prolactin may come with milk-like ni**le discharge. Thyroid disease can disturb cycle timing. Premature ovarian insufficiency may bring hot flashes and high FSH. Previous uterine procedures may point toward an outflow problem such as intrauterine adhesions.

That is why missed periods should not be treated as “just hormones” or ignored for months. The basic workup often starts with a pregnancy test, then targeted labs such as TSH, prolactin, FSH/LH, and sometimes androgen testing or pelvic ultrasound depending on the clinical picture.

The key message: a missed period is a symptom, not a diagnosis. The body is not being random — it is giving a signal. The real skill is knowing which clue to follow first. 🩺
