The fluid mismatch sign for LHBT tenosynovitis. How do you confidently call biceps tenosynovitis on a non-contrast MRI? Look for a mismatch. A localized fluid collection in the long head of the biceps tendon sheath combined with a completely dry glenohumeral joint (zero effusion) is a massive red flag for tenosynovitis.

Speed is the second most common problem radiologists face with MSK MRI reporting. It gets harder because of a limiting belief some radiologists hold: that fast reporting is dangerous, or that reporting speed is some intrinsic quality you either have or you don't. It is not. Slow reporting is a symptom. The cause is a lack of confidence, a lack of a systematic approach, or both.

Radiologists in my Virtual MSK Fellowship saw their reporting speed increase by an average of 3.82x after a year of training with me. I can teach you how to at least double your speed, with better quality. Comment "MSK" and I’ll DM you the link to book a free strategy session.

Diagnosing the hidden causes of ulnar-sided wrist pain. Months after a minor wrist contusion, this patient still has pain. The key finding is subtle: an abnormal, attenuated joint capsule and thickened ligaments between the hamate and the 5th metacarpal base. This is a classic 5th CMC joint distortion. Always scrutinize these ligaments on both your coronal and axial views to catch what others miss.

Upgrading your diagnostic terminology. In this case, we have a hip with severe osteoarthritis, massive bone marrow edema, and a large effusion. What is the true cause of the acute pain? Switch to the sagittal plane. There is a distinct subchondral insufficiency fracture. Because we see characteristic medial head sparing, we know this is not AVN. We need to stop calling this "transient osteoporosis of the hip." (Search for my Vancouver Bone Squad lecture on YouTube to learn more)
