Starting HRT at 50 is not the same as at 70?

The WHI concluded that the best time
to start your hormone therapy is

- before the age of 60 or
- within the first 2 years of menopause

For post menopausal women - within the first 10 years after your last menstrual cycle appears to lower risk of developing age related conditions: cardiovascular, metabolic, genitourinary, brain conditions etc.

The Women’s Health Initiative (WHI) changed the way millions of women — and many doctors — viewed hormone therapy.

But more than 20 years later, one thing is clear: the findings need context.

The WHI identified important risks associated with the specific hormone therapy studied. But it also showed benefits — and overall mortality was not increased.
One of the most important details?

🎥 Part 1: Who was treated?

- Women aged 50–79, with an average age of 63 — many were considerably older than the age at which hormone therapy is typically prescribed today for menopausal symptoms and age-related prevention.

The major less is that hormone therapy cannot be considered in isolation:

- Age, timing, medical history, individual risk factors — and, as we’ll discuss next, the type of hormones and treatment used — all matter.

“Hormone therapy causes cancer.”

The Women’s Health Initiative (WHI) study in the early 2000s dramatically influenced how women — and many healthcare professionals — thought about menopausal hormone therapy.

What often gets missed is that hormone therapy isn’t one single treatment, and its potential risks and benefits cannot be separated from factors such as:
• The type of hormone therapy
• Which hormones are used
• The dose and route of administration
• Duration, age and timing
• Individual health and risk factors

Understanding the evidence is far more useful than making decisions based on a headline from decades ago.
