E/A above 2 in the evaluation of diastolic dysfunction in a „sick“ left ventricle denote restrictive filling and therefore elevation in filling pressures. So the previous signal we saw with an S wave that was smaller than the D wave and the E/A above 2, both show that this patient has significantly elevated filling pressures.

Left ventricular filling pressures are sometimes hard to interpret. In this case you can see a pulmonary venous signal:
- PW-Doppler sample volume should be 2mm in size (not 5 or more as we are used to in TTE)
- SD
- In a super normal filling S can be < than the D wave, the differentiation to elevated filling pressures is the diseased LV vs athletes heart
