📚 EUSOBI Paper of the Week

CEM vs MRI for assessing ni**le-areolar complex involvement

NAC involvement is an important consideration when planning ni**le-sparing surgery. But can CEM provide an assessment comparable to MRI?

This retrospective study compared 91 CEM and 104 MRI examinations, using histopathology as the reference standard.

🔎 Key findings:
• Comparable diagnostic performance
• Specificity ~96% for both modalities
• High inter-reader agreement
• Different independent imaging predictors for CEM and MRI

💡 Bottom line:
CEM may be a useful alternative to MRI for NAC assessment in appropriately selected patients - but further prospective validation is needed.

This week, we are featuring the new ACR Appropriateness Criteria on breast imaging during lactation, providing clear recommendations across common clinical scenarios.

🍼 Breast imaging during lactation should not be delayed.

Physiological changes during breastfeeding can make breast assessment more challenging, but they should never prevent appropriate imaging when clinically indicated.
The new ACR Appropriateness Criteria® provide practical, evidence-based recommendations for breast imaging during lactation, covering common clinical scenarios such as screening, palpable masses, ni**le discharge, breast infection, and staging of newly diagnosed breast cancer.
Key messages include the safe use of mammography, ultrasound, and contrast-enhanced MRI when appropriate, while supporting the continuation of breastfeeding whenever possible.

This week, we're featuring a study that highlights the growing potential of breast imaging beyond breast cancer detection.

🫀 Can routine mammograms help identify women at increased cardiovascular risk?

The authors demonstrate that AI-based quantification of breast arterial calcifications (BAC) from screening mammograms is a strong independent predictor of major adverse cardiovascular events and adds prognostic value beyond established cardiovascular risk scores.

The study underscores the potential of opportunistic screening, showing how breast imaging may contribute not only to early cancer detection but also to improving cardiovascular prevention and women's health.

Congratulations to the authors on this important publication, and thank you for advancing interdisciplinary research at the intersection of breast imaging, artificial intelligence, and cardiovascular medicine.

Paper of the Week 📄

This week, we’re highlighting a publication from The Lancet reporting the primary interval cancer outcomes from the MASAI trial, one of the largest randomized studies evaluating AI-supported mammography screening.

The study assessed whether AI-supported screening could maintain screening safety while improving screening performance compared with standard double reading.

Key findings include:
🔹 Non-inferior interval cancer rates compared with standard double reading
🔹 Higher sensitivity with unchanged specificity
🔹 Fewer invasive, T2+, and non-luminal A interval cancers
🔹 Reduced radiologists’ reading workload

These findings add important evidence on the role of AI in population-based breast cancer screening and contribute to the ongoing discussion on its potential implementation in clinical practice.
