Learn about Common Peroneal Nerve Entrapment!

Common peroneal nerve entrapment is a condition where the common peroneal nerve becomes compressed or irritated, most commonly around the outside of the knee near the fibular head. This nerve helps control movement of the ankle and toes and provides sensation to parts of the lower leg and foot. Entrapment can cause symptoms such as pain, tingling, numbness, or weakness in the outer lower leg and top of the foot. In more severe cases, it can lead to difficulty lifting the foot and toes, sometimes resulting in foot drop and an altered walking pattern.

Common peroneal nerve entrapment can occur due to prolonged pressure around the outside of the knee, direct injury, trauma, or repeated movements that place stress on the nerve. It may also develop following fractures or other injuries around the knee, prolonged leg crossing, tight casts or braces, or conditions that cause swelling or compression. Reduced muscle strength, poor movement patterns, and previous injuries may also contribute to nerve irritation and functional difficulties.

Treatment focuses on reducing pressure on the nerve, managing symptoms, and restoring normal movement and function. Physiotherapy can play an important role through exercises to maintain or improve strength, flexibility, balance, and walking ability. Nerve mobilisation techniques may be used when appropriate, alongside education about avoiding prolonged positions or activities that increase nerve compression. Physiotherapists may also recommend gait training, footwear or orthotic support, and strategies to reduce the risk of falls when weakness or foot drop is present.

In some cases, medical management, further investigations, or surgery may be required if symptoms are severe, persistent, or caused by significant compression.

Talar dome fractures are injuries to the smooth, cartilage-covered surface of the talus, the bone that forms the lower part of the ankle joint. These fractures often occur after a significant ankle injury, such as a severe sprain, twisting injury, fall, or motor vehicle accident. Because the talar dome is responsible for transferring weight through the ankle and is covered by articular cartilage, these injuries can lead to ongoing pain, stiffness, swelling, and difficulty walking if not properly managed.

Talar dome fractures occur when excessive force damages the bone and the overlying cartilage. In some cases, a fragment of bone and cartilage may become displaced, while in others the injury may be subtle and difficult to detect on initial X-rays. The talus has a relatively limited blood supply, which can slow healing and increase the risk of complications such as delayed union, non-union, avascular necrosis, or the development of post-traumatic ankle arthritis. These factors can make talar dome fractures challenging to treat and may prolong recovery.

Management depends on the size, location, and severity of the fracture. Stable injuries may be treated with a period of immobilisation and restricted weight-bearing to allow the bone to heal, while larger or displaced fractures may require surgery to restore the joint surface and improve stability.

Physiotherapy plays an important role throughout recovery by helping to restore ankle range of motion, strength, balance, and walking ability while gradually progressing weight-bearing as healing allows. Rehabilitation also focuses on reducing stiffness, improving function, and supporting a safe return to daily activities and sport where appropriate.

Early diagnosis, appropriate treatment, and a structured rehabilitation program can help optimise recovery and reduce the risk of long-term complications.

Proximal hamstring tendinopathy is a condition that affects the hamstring tendon where it attaches to the sitting bone (ischial tuberosity). It causes pain deep in the lower buttock, particularly during activities such as running, prolonged sitting, climbing hills, or bending forward. The condition is most common in runners and other physically active individuals but can also occur in people whose daily activities place repeated strain on the hamstring tendon.

Proximal hamstring tendinopathy develops when the tendon is exposed to repeated or excessive load without sufficient time to recover. Contributing factors include sudden increases in training intensity or volume, inadequate muscle strength, poor movement patterns, prolonged sitting, and reduced flexibility or control around the hips and pelvis. In some cases, previous hamstring injuries or inadequate recovery from exercise may also increase the risk of developing the condition.

Management focuses on reducing pain, improving tendon capacity, and gradually returning to normal activity. Physiotherapy plays an important role through a progressive strengthening program targeting the hamstrings and surrounding muscles, as well as exercises to improve pelvic stability, movement control, and flexibility where appropriate. Education on activity modification and gradual return to sport or exercise is also important. Other management strategies may include temporarily reducing aggravating activities, addressing training errors, and maintaining an appropriate level of physical activity. With appropriate rehabilitation and gradual progression of loading, most people can return to their usual activities with reduced pain and improved function.

Learn about Frozen Shoulder!

Frozen shoulder, also known as adhesive capsulitis, is a condition that causes pain, stiffness, and reduced movement in the shoulder joint. It typically develops gradually and can make everyday activities such as reaching overhead, dressing, or lifting objects difficult. Frozen shoulder most commonly affects adults between the ages of 40 and 60 and is more common in women. It is also more likely to occur in people with diabetes, following a shoulder injury or surgery, or after a period of prolonged shoulder immobilisation.

Frozen shoulder occurs when the capsule surrounding the shoulder joint becomes inflamed, thickened, and tight, leading to the formation of scar tissue that restricts movement. The exact cause is not always known, but risk factors include diabetes, thyroid disorders, prolonged inactivity after injury or surgery, and previous shoulder conditions. The condition typically progresses through four stages: the pre-freezing stage, where shoulder pain begins but movement is only mildly affected; the freezing stage, where pain increases and shoulder stiffness gradually worsens; the frozen stage, where pain often settles but stiffness is at its greatest and movement is significantly restricted; and the thawing stage, during which shoulder mobility slowly improves and function gradually returns. Recovery can take several months to up to two years.

Management focuses on reducing pain, restoring shoulder mobility, and improving function. Physiotherapy plays an important role through gentle stretching, range of motion exercises, strengthening programs, and manual therapy to improve joint mobility and restore normal movement. Education about activity modification and maintaining shoulder movement within comfortable limits is also important. Other management strategies may include pain-relieving medications, anti-inflammatory medication, corticosteroid injections, and, in persistent cases, procedures such as hydrodilatation or surgery. Early assessment and appropriate treatment can help improve recovery, reduce pain, and restore shoulder function.

This can cause symptoms such as numbness, tingling, pain, and weakness in the hand and fingers, particularly the thumb, index and middle finger. The condition can affect people of all ages but is more common in individuals who perform repetitive hand and wrist movements as part of their work, hobbies, or daily activities. Symptoms often develop gradually and may worsen at night or during activities that involve prolonged gripping or wrist positions.

Carpal tunnel syndrome occurs when pressure within the carpal tunnel increases, placing stress on the median nerve. Contributing factors may include repetitive hand use, prolonged or awkward wrist positions, wrist injuries, pregnancy, diabetes, arthritis, and other conditions that cause swelling around the wrist. In some cases, the exact cause is unclear. Over time, ongoing nerve compression can lead to reduced hand strength, difficulty performing everyday tasks, and persistent symptoms if left untreated.

Management focuses on reducing pressure on the median nerve, relieving symptoms, and improving hand function. Physiotherapy plays an important role through exercises designed to improve nerve mobility, wrist and hand flexibility, strength, and overall function. Treatment may also include education on activity modification, ergonomic adjustments, and strategies to reduce strain on the wrist during daily tasks. Other management options may include wrist splinting, anti-inflammatory medications prescribed by a doctor, or corticosteroid injections. In severe cases, surgery may be recommended to relieve pressure on the nerve. Early diagnosis and appropriate management can help reduce symptoms and prevent long-term complications.
