Hunting for a canal that looks completely obliterated on the radiograph? Troughing wildly with a round but is likely to land you in perf territory. The best course of action is to pause and study the anatomy, the radiographs, follow the dentine map and trough judiciously with specialised burs and ultrasonics.

The endless endo debate usually comes down to two philosophies: total control with progressive shaping, or maximum efficiency with single-file reciprocating systems.
With newer conservative files entering the mix, the lines are blurring even more. But at the end of the day: irrigation, anatomy respect, and technique will always matter more than the brand of metal in your handpiece.
It’s the archer, not the arrow.

This or that - periapical surgery edition; Early on, I thought a tiny incision meant conservative surgery.

Instead, it meant 40 minutes of fighting tension, poor visibility and tearing tissue edges just trying to reach the apex.

And the worst part? It could have been completely avoided with better planning before I picked up the scalpel.

Now, I spend more time planning the surgery before I enter the operatory. I study the CBCT, thinking about access and choosing the flap that actually fits the anatomy.

And things go a whole lot smoother.
