Brisbane North PHN Helping our community to access the right care, in the right place, at the right time.

Brisbane North PHN supports clinicians and communities in Brisbane’s northern suburbs, Moreton Bay Regional Council and parts of Somerset Regional Council. It covers approximately 4,100 km2 of urban, regional and rural areas, with a population of over 900,000. The Federal Government announced the establishment of 31 Primary Health Networks to replace the national network of Medicare Locals in 2015

. The Government selected successful PHNs through an open and competitive process, including the Brisbane North PHN. The key objectives of the Brisbane North PHN are:
• increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
• improving coordination of care to ensure patients receive the right care in the right place at the right time. The PHN will achieve these outcomes by:
• understanding the health care needs of our community through analysis and planning, helping to identify and address service gaps;
• providing practice support services so that GPs are better placed to provide care to patients, keeping them healthy and out of hospital;
• supporting general practices in attaining the highest standards in safety;
• assisting general practices in understanding and making meaningful use of eHealth systems;
• working with other funders to purchase or commission health services for local groups most in need, including patients with complex chronic conditions or mental illness. The PHN supports GPs and other health professionals, and delivers a range of programs. Supporting health care professionals with:
• localised clinical pathways
• electronic referrals
• accreditation and quality standards
• chronic disease management and care coordination
• eHealth records and secure messaging. Managing healthcare and support programs for people in the community including:
• Commonwealth Home Support Program
• Partners in Recovery
• Brisbane MIND, Brisbane MIND Suicide Prevention Program, Brisbane MIND4KiDS
• Team Care Coordination

🎊 Celebrating 30 years of Team Care Coordination 🎊

Brisbane North PHN’s Team Care Coordination is celebrating three decades of delivering support for people with chronic and complex health needs across North Brisbane and Moreton Bay.

First established as a GP-led pilot in the late 1990s, the program has grown from a local care coordination model into a trusted, multidisciplinary service.

In the last financial year alone, Team Care Coordination supported 2,882 clients to manage their health, access appropriate services and stay safely and confidently at home.

Are you familiar with the healthcare needs of our region? Do you live locally? We are seeking expressions of interest from community members to join our Community Advisory Committee (CAC).

Our CAC is a key forum to bring together diverse perspectives across a range of population groups, playing an essential role by assisting the PHN with strategic input, planning and communications.

We are particularly interested in receiving applications from young people aged 18-25 to ensure broader representation, however, all ages are encouraged to apply.

With accreditation readiness and the use of AI becoming increasingly important topics for general practice teams, Brisbane North PHN recently held a networking session, supporting practices to stay informed and connected. 📋 🖥️

Attendees said the event improved their knowledge across several areas, including accreditation expectations, upcoming changes to the RACGP standards, AI consent, AI use in general practice, 1800 Medicare, practice systems and sustainability. Feedback also showed strong interest in future education and networking opportunities.

We use this feedback to help shape future education and networking opportunities for general practice teams across the region. Keep an eye on our events calendar on our website where we list all of our PHN events. 📅

Heading to the Ekka? 🎡🍦🍓

We’re proud to see Brisbane North PHN Board Director Dr Kelly Lai supporting the community as one of the specialist GPs working at the Ekka Flu Vaccination Clinic.

The clinic is offering free influenza vaccinations this week from 9.00 am–2.00 pm.

Receiving a flu vaccination remains one of the most effective ways to reduce the risk of serious illness from influenza. That’s something for the whole family. ❤️

The second meeting of the Metro North DBT Collaborative was held recently at Brisbane North PHN’s offices in Chermside.

Some of the practical ideas discussed were:
👥 creating shadowing arrangements between Hospital and Health Services and community providers
🔄 sharing facilitators across services
🤝 supporting peer supervision through Metro North
🔎 exploring whether a dialectical behaviour therapy (DBT) consulting group could help strengthen capability and consistency across the region.

It’s wonderful to see the Collaborative continue to grow and we look forward to the next meeting at Redcliffe Medicare Mental Health Centre’s newly refurbished premises on Ningi Ningi Country.

Today is Aged Care Employee Day – an opportunity to share our gratitude for the many people caring for and supporting older people in our community who receive aged care services. 🫶

From nurses, personal care workers, chefs, cleaners, gardeners, laundry staff, activity coordinators and administration staff – each plays a key role in the lives of older people and their families, offering not only medical and physical care but also emotional care and companionship.

Peter Melrose, Brisbane North PHN Manager – Connected Care, said the aged care workforce plays a vital role in helping older people in our region remain healthy, connected and supported.

“Today, at Brisbane North PHN, we're taking a moment to celebrate the commitment, expertise and kindness aged care employees bring to their work and acknowledge the positive impact they have on the lives of so many valued people in our community,” he said.

Brisbane North PHN works closely with residential aged care homes our region, supporting staff and partnering to improve access to high-quality care for residents.

Two general practice nurses in the Brisbane North PHN region are helping drive meaningful improvements in preventive care. 👩‍⚕️👩‍⚕️

Through their involvement in PHN programs, both nurses have contributed to increased vaccination uptake, improved participation in cancer screening programs, enhanced patient data quality and more culturally responsive care.
