FATTY LIVER? HAVE YOU LOOKED AT YOUR GUT? 🦠

One of the most overlooked contributors to metabolic liver inflammation is what’s happening in the gut.

And specifically, the role of LPS (lipopolysaccharides).

LPS are inflammatory endotoxins found in the outer membrane of gram-negative bacteria.

Here’s why this matters.

Your gut and liver are directly connected through the portal circulation.

When the gut microbiome is disrupted and intestinal barrier function is compromised, greater amounts of bacterial products like LPS may reach the liver.

Once there, LPS can activate inflammatory pathways, including Kupffer cells and TLR4 signalling contributing to:

🔥 Hepatic inflammation
🔥 Impaired insulin signalling
🔥 Altered lipid metabolism
🔥 Progression of metabolic fatty liver disease (MASLD)

Fatty liver isn’t simply a liver problem.

It’s often part of a much bigger metabolic picture involving insulin resistance, visceral fat, lipid metabolism and potentially gut dysbiosis.

This is why I investigate fasting insulin, triglycerides, glucose regulation, liver markers, bowel function and, where appropriate, the gut microbiome.

Stop looking at organs in isolation.

Start understanding how your systems communicate.

This is the work we do inside my 12-Week Hormone Nurse Intensive or book a one off appointment and we can get your testing organised

Your s*x hormones are declining. But what is happening to your insulin?

This is one of the biggest metabolic shifts I see in women during perimenopause and menopause. And it’s often overlooked.

We talk about declining oestrogen and progesterone, but rarely about how these changes affect insulin sensitivity.

Insulin isn’t just your fat-storage hormone. It also helps your body access energy.

When your cells become less responsive to insulin, everything can start to feel harder.

As oestrogen declines and fluctuates, we can see changes in:

→ Insulin sensitivity — oestrogen helps regulate how your cells respond to insulin.

→ Fat distribution — more abdominal and visceral fat, which is linked to insulin resistance.

→ Muscle mass — less metabolically active muscle means reduced glucose disposal.

→ Sleep and cortisol — disrupted sleep and stress can worsen insulin sensitivity.

→ Appetite and energy — more cravings, fatigue and difficulty managing weight.

And here’s where the cycle begins.

Your body produces MORE insulin to keep blood glucose stable.

Persistently elevated insulin promotes fat storage and can make accessing stored fat harder.

Increasing visceral fat can then worsen insulin resistance.

This is why I see women eating less, exercising more and STILL struggling with their weight.

It’s not always about calories.

It’s about how efficiently your body manages and uses energy.

And it’s why another restrictive diet often misses the bigger picture.

We need to address insulin, muscle, sleep, stress, nutrition and hormones TOGETHER.

This is exactly what we work on inside my 12-Week Hormone Nurse Intensive.

We investigate what’s driving your symptoms, assess your metabolic and hormonal health, and create an individualised plan to support insulin sensitivity, energy and metabolic function.

Not another quick fix.

A deeper understanding of what your body needs during this transition.

The longer I do this work, the more I realise how deeply our programming influences our results.

People come to me knowing so much.

They’ve read the books. Seen the specialists. Tried the supplements. They can tell me exactly what they “should” be doing.

But knowing what to do and being able to consistently do it are two very different things.

Sometimes underneath the struggle is a belief:

“I’ve always been this way.”
“Nothing works for me.”
“Everyone else needs me first.”
“If I can’t do it perfectly, why bother?”

Sometimes being busy feels familiar. Rest brings guilt. Looking after yourself feels selfish.

And sometimes the plan simply asks more of you than your life currently has room for.

This is why I’m becoming more interested in the person trying to follow the plan.

What do they believe is possible for themselves?
What happens on the hard days?
What makes their own needs so easy to abandon?

Knowledge matters. So do your symptoms, your circumstances and the support you have.

But another protocol can only take you so far if we never explore what keeps getting in the way.

You can know exactly what to do and still need help making it something you actually live.

It’s AMAZING what the colon can hold. 💩

But the real gold isn’t just what comes OUT.

It’s what’s happening BEHIND the poo.

Behind your colon sits an incredible network of blood vessels, lymphatics and nerves, all connected to your gut, liver and wider metabolic health.

The mesenteric blood supply is particularly fascinating.

Blood draining from your intestines travels through the portal circulation directly to your liver, where nutrients are processed and substances absorbed from the gut can be metabolised and cleared.

And this is why I always say…

Your colon is so much more than a place to store waste.

It’s part of an interconnected system involved in digestion, immune function, microbial metabolism and waste elimination.

When bowel movements are sluggish, it’s worth looking beyond the poo.

Think gut motility, hydration, fibre, microbial activity, inflammation and liver function.

And while colonics can help evacuate bowel contents, they don’t directly cleanse the blood or detoxify the liver.

The real gold is understanding the physiology behind the poo. ✨

You keep blaming your hormones. But have you looked at the environment they’re trying to function in? 👀

Your hormones don’t operate in isolation.

They’re constantly responding to signals from your gut, metabolism, immune system and nervous system.

And when that internal environment is under stress, your hormones can feel the consequences.

Here are 3 BIG drivers of inflammation I look at as a Hormone Nurse. 👇

1️⃣ INSULIN RESISTANCE

Insulin isn’t just about blood sugar or weight gain.

Chronically elevated insulin and insulin resistance are closely linked with low-grade inflammation, altered ovarian hormone signalling and metabolic dysfunction.

And insulin resistance isn’t always just about what you’re eating.

Poor sleep, chronic stress, low muscle mass, inactivity and genetics all play a role.

2️⃣ GUT HEALTH

Your gut is home to a huge proportion of your immune activity.

When the gut barrier is compromised or the microbiome is disrupted, inflammatory signalling can increase.

Constipation, poor dietary diversity and certain gut conditions can add to the picture.

Your gut isn’t just about digestion. It’s communicating with your immune system every single day.

3️⃣ CHRONIC STRESS & POOR SLEEP

Your nervous system and immune system are constantly communicating.

Persistent stress and disrupted sleep can alter cortisol rhythms, impair glucose regulation and increase inflammatory signalling.

And here’s the kicker…

Poor sleep can worsen insulin resistance, which can further fuel metabolic inflammation.

SEE HOW IT’S ALL CONNECTED?

Your gut. Your insulin. Your cortisol. Your immune system. Your hormones.

They’re not separate systems.

So before we blame estrogen, progesterone or perimenopause for absolutely everything, perhaps we need to look at the environment those hormones are functioning in.

Because sometimes the hormones aren’t the starting point. They’re part of the bigger picture.
