That old ache that gets louder this time of year? You're not imagining it.

Cold weather doesn't really cause new pain. It just stops hiding what was already there. Less movement, more time at a desk, colder muscles, and the ni**le you'd been getting away with starts to speak up.

We see a lot of patients around this time of year coming in to get an old issue treated that recently got a bit worse.

The sore spot is rarely where the problem started. A cranky knee, a tight shoulder, a stiff neck is often being driven by something further up or down the body that isn't pulling its weight. So we look at how the whole body is moving, not just the bit that hurts. And we don't sign you up for months of visits. We find what's driving it, work on that, and leave you with a plan you can run with.

If something's been nagging you this winter, it's a good time to get it looked at.

So why would a rib be behind that stubborn spot? Here's what we see all the time.

Your ribs don't just wrap around the front of you. They attach onto your spine at the back, right where that knot sits between your shoulder blade and your spine. Each of those little joints is meant to move a bit every time you breathe. When one gets stiff and stops gliding, the area around it gets tight and achy, and massaging the muscle on top of it never quite fixes it. That's why it keeps coming back.

It also rarely sits on its own. That part of your upper back is where your neck, your shoulder blade, your ribs and your breathing all meet. When your core and diaphragm aren't controlling your breath well, the muscles around your neck and upper back start working overtime, and those rib joints cop the extra load. So the tight spot is usually the loudest part of a bigger pattern, not the whole story.

What actually helps is getting the rib moving again with a gentle adjustment, settling the muscles that have been guarding it, and sorting out how you breathe and brace through your core so it stops getting overloaded.

If there's a spot back there you can never quite get rid of, it's worth having the rib and your breathing looked at, not just the muscle.

Note: this is general education, not a diagnosis. If something's sharp, ongoing or not settling, come and get it checked.

Your upper back, neck and shoulders always tight? You've probably blamed posture or stress. But there's a piece most people never check: how you breathe.

You breathe around 20,000 times a day. Ideally your diaphragm does most of that work, with your deep core supporting it underneath. When that control slips (stress, long days at a desk, a habit of shallow chest-breathing), your body calls in the backup team: the accessory breathing muscles in your neck. The scalenes, upper traps and levator scapulae start helping you breathe, thousands of times a day.

They aren't built for that. So they get overworked, tight and sore, and that shows up as the "my shoulders are always tight" feeling we hear all the time. It's also why a stretch or a massage only lasts a day. It calms the muscle, but it doesn't change why it's working so hard.

Most of those tight muscles also attach to ribs, which means that the joints that move those ribs get irritated, resulting in a "stuck rib". This feels like a pain behind the shoulder blade and often comes with difficulty breathing.

The real fix is usually further down: getting your core and diaphragm controlling your breath again, so your neck can finally clock off breathing duty. Even if we adjusted the rib but didn't address the muscles, the rib would come back.

If you have chronically tight shoulders, a pain behind your shoulder blade, or difficulty breathing (and aren't suffering from a heart attack), consider coming in so we can get to the bottom of it for you.

Foam rolling, ball work, massage gun - most people are doing the technique, but missing the one thing that actually makes a muscle let go. 🎯

Here's the method we use, and it works on any muscle with any tool:

1️⃣ Explore the whole muscle first. Roll slowly and hunt for the genuinely tender spots - don't just grind up and down.
2️⃣ Park on the sore spot, but stay on the EDGE. Enough to feel it, not so much you can't relax. Drive too deep into pain and your body braces — and a braced muscle won't release.
3️⃣ Breathe, wait 10–20 seconds, then FOLLOW it. As the tenderness fades, sink a little deeper to the new edge. That "following" is where the change happens.

Same rules for your legs, back, neck, shoulders and feet. Use a firm ball for small spots (skip the spiky ones - that sharp skin pain just stops you going deep enough), a foam roller for big areas like the thighs, or a massage gun - they all run on the same principles.

Bonus: the best time to find these spots is a quick daily body-scan. Tune in head to toe and compare left to right. The stiffer side is usually where to start.

This is general self-care, not a diagnosis. Anything sharp, shooting, numb, or that won't settle - get it looked at. And if the same knot keeps coming back no matter how much you release it, that's your body telling you something upstream is driving it - come see us and we'll find why.

Got sore knees or hips? It might be the ITB thats the culprit.

These are particularly pesky problem areas for athletes that run a lot as the Itb, hip and knee are all needed to stabilise your pelvis when you run.

Treatment is not usually as simple as rolling or massaging the outside of your leg. We often find the glutes, hip flexors, groin and even the shin muscles are part of the problem and getting tested properly in all these areas is key.

The joints of the pelvis, hip, knee and ankle need to all be checked for movement dysfunction as well.

If you have ITB problems, don't let it turn into something more serious!
