Because β€œI watched a video about it” isn’t always enough. πŸ˜‰

Sometimes, you need to learn it. Practise it. Ask questions. Do it properly.

🏠 Before the first shift, a family should not be left wondering who is coming or what happens if something changes.

πŸ“‹ Before services begin, participants and families should receive clear information about:
πŸ—“οΈ the schedule
πŸ‘€ worker introductions (where possible)
πŸ“ž contact points
🀝 agreed supports
πŸ’³ fees and travel arrangements
πŸ”„ cancellations
πŸ’¬ feedback and complaints
πŸ”’ privacy
🚨 incident response
πŸŒ™ after-hours escalation

🩺 For complex care, they should also know that workers have been prepared for the person’s specific needs and understand the current plan.

πŸ’‘ A welcome process is not administrative decoration. It reduces uncertainty, protects choice, and gives the participant a clearer voice from the beginning.

πŸ”– Save this list when comparing or beginning with a new provider.

🧩 Not every discharge delay is clinical. Sometimes the missing piece is coordination.

🚧 Complex discharges can stall when the proposed roster is unclear, equipment has not arrived, worker competencies are incomplete, funding or service details are uncertain, the home has not been assessed, or key documents are still moving between teams.

πŸ—ΊοΈ The earlier a provider is included, the sooner these dependencies can be mapped. That does not mean promising a start before safety is confirmed. It means identifying what is ready, what is missing, who owns each action, and which items are critical before discharge.

πŸ’‘ A transparent readiness conversation can save time, reduce last-minute pressure, and protect the participant from an unsafe or confusing transition.

🀝 Planning a complex discharge? Invite us into the conversation early.

Not all care is created equal. And neither is training.

A general training certificate can give you knowledge.
The right clinical training can give you confidence in the moments that matter.

From diabetes and medication management to complex bowel care, wound care, enteral feeding, epilepsy and urinary catheter management, Flonac's clinical education workshops are designed around the realities of complex care.

Because when you're supporting someone with complex needs, β€œI think I know how” isn't the standard.

Know the skill. Understand the risk. Deliver better care.

πŸ›‘οΈ Allied health collaboration is a safety issue, not just a nice-to-have.

πŸ‘₯ A safe transition depends on what happens between disciplines, not only within them. Physiotherapists, occupational therapists, speech pathologists, dietitians, nurses, doctors, support workers, Support Coordinators and families may all hold a different part of the person’s story.

πŸ’‘ When information is shared well and roles are clear:
βš™οΈ equipment is used more safely
πŸ’¬ communication strategies stay consistent
πŸ₯— nutrition and swallowing plans are followed
🚨 changes are escalated to the right person

🎯 Collaboration is not another meeting for the sake of it. It is making sure the person receives one coordinated approach rather than several disconnected plans.

🏷️ Tag an allied health professional whose collaboration makes a real difference.

You can care deeply about the people you support and still have questions about complex care.

What matters is recognising that learning is part of providing good care.

Clinical knowledge grows through education, practice, and the confidence to keep asking questions.

At Flonac Academy, our workshops are designed to help healthcare professionals strengthen their clinical knowledge through practical, evidence-based education.

Because confidence is not about knowing everything.

It is about being prepared to keep learning.

πŸ“‹ Referral-ready information (for complex care)

βœ… A complete referral helps the right provider say yes safely and sooner. For complex support, include these essentials where consent allows:

🎯 Participant goals and preferred communication
🩺 Diagnosis and current clinical summary
πŸ‘€ Functional needs and daily routines
⚑ Required high-intensity supports
πŸ’Š Equipment, consumables and medication information
⚠️ Known risks and escalation instructions
🧠 Behaviour support information, if relevant
πŸ“ Home access, location and proposed schedule
πŸ“… Key contacts and anticipated discharge date

🀝 Missing information does not mean a referral cannot begin. It does mean the provider and referrer should agree on what must be obtained before services safely commence.

πŸ“„ Clear information reduces back-and-forth and helps everyone focus on the person, not the paperwork.

The moment a participant needs more than routine support, preparation matters.

A new clinical need.
A change in condition.
A discharge plan that requires more than general training.

For healthcare professionals working across disability, community and aged care, confidence comes from understanding the specific skills requiredβ€”and knowing how to apply them safely.

That is where Flonac Clinical Education comes in.

Our practical, evidence-based workshops are designed to strengthen clinical knowledge and build confidence in areas such as:

Complex Bowel Care & Stoma Management
Complex Wound Care Management
Diabetes Management
Enteral Feeding
Epilepsy Management
Medication Management
Urinary Catheter Management

Whether you are a Registered Nurse, Community Nurse, Support Worker, Assistant in Nursing, Disability Support Professional, NDIS provider or healthcare organisation, the right education can make a meaningful difference to the quality and safety of care.

Not just more training. More prepared people, stronger clinical practice and better support for participants.
