💳 fees and travel arrangements
🔄 cancellations
💬 feedback and complaints
🔒 privacy

🌙 after-hours escalation

🩺 For complex care, they should also know that workers have been prepared for the person’s specific needs and understand the current plan.

💡 A welcome process is not administrative decoration. It reduces uncertainty, protects choice, and gives the participant a clearer voice from the beginning.

🧩 Not every discharge delay is clinical. Sometimes the missing piece is coordination.

🚧 Complex discharges can stall when the proposed roster is unclear, equipment has not arrived, worker competencies are incomplete, funding or service details are uncertain, the home has not been assessed, or key documents are still moving between teams.

🗺️ The earlier a provider is included, the sooner these dependencies can be mapped. That does not mean promising a start before safety is confirmed. It means identifying what is ready, what is missing, who owns each action, and which items are critical before discharge.

💡 A transparent readiness conversation can save time, reduce last-minute pressure, and protect the participant from an unsafe or confusing transition.

💊 Equipment, consumables and medication information
⚠️ Known risks and escalation instructions

🤝 Missing information does not mean a referral cannot begin. It does mean the provider and referrer should agree on what must be obtained before services safely commence.
