Tomorrow is a big day for all those sitting the exam to become an International Board Certified Lactation Consultant

Becoming an IBCLC is no small undertaking, particularly when you’re already working as a health professional. It requires 95 hours of lactation-specific education, 1,000 hours of lactation-specific clinical experience, and a 4-hour international board examination.

And it doesn’t stop once you pass. IBCLCs commit to ongoing professional education, maintaining clinical skills and professional standards, appropriate insurance and recertification every 5 years.

It’s a significant investment of time, study and commitment and ultimately means having another level of knowledge and skills to support women, babies and families with infant feeding.

I would like to acknowledge Hailee and everyone sitting the exam and all the work they have put in to get to this point.

I believe every woman should have access to information, education and support that is evidence-based, balanced and personalised.

I do my best, both here and through my business, to provide exactly that.

But I also know there are so many women who live remotely, don’t know what support is available, or simply haven’t found the right person to help them yet.

The first thing I do when I meet a couple in any situation around pregnancy, birth or postpartum is ask:

How are you?
How are you feeling?
Why are you here?
What have you been told?
What would you like?

Then I explain….

What I’m doing.
What the machines are doing.
What the plan looks like.
Why we’re recommending something.
And I ask if they’re comfortable with it, if they consent, and if they have questions.

This is so important.

It’s not our journey. It’s not our job to dictate.

Our job is to explain, recommend, gain consent and work with women and families.

It really is that simple.

A woman’s body is hers. We can explain the risks, recommendations and policies, but she still has the right to ask questions, decline and decide what, why and how.

I GET ASKED THIS MULTIPLE TIMES A WEEK

This is what I say…

I don’t have a problem with them, and quite frankly, when parents are informed of risks and benefits, they should be supported to do what works best for their family.

Some babies have a really strong need to suck, and for those babies a dummy can be a fantastic tool. It can help with settling and soothing, and there’s good evidence that using a dummy during sleep is associated with a reduced risk of SIDS.

My biggest advice? Feed first. Dummy second.

Especially in the early weeks, always make sure your baby has been offered a feed, is feeding well, gaining weight appropriately, has good wet and dirty nappy output, and your milk supply is well established before using a dummy to settle them.

A dummy should never replace a feed or be used to delay feeds. Babies suck for both nutrition and comfort, and both are important. Missing feeding cues can affect weight gain and milk supply.

There are a few things to keep in mind:

• Introducing a dummy before breastfeeding is established may contribute to breastfeeding challenges for some babies.

• Long-term dummy use (particularly beyond 2–4 years) can affect dental development and palate shape.

• Keep dummies clean, replace them if they become worn, and never dip them in honey or other sweet substances.

For me, a dummy is a tool. Used at the right time, for the right reasons, it can be incredibly helpful for some babies and families.

As always this is general advice and always talk to your healthcare provider for individualised advice for you and your baby.

WHY THE KOALA HOLD CAN BE BENEFICIAL

1. Uses gravity to help with swallowing and milk flow

- Baby is more upright, which can make it easier to coordinate sucking, swallowing and breathing.

- It can be particularly helpful for babies who cough, splutter or struggle with a fast let-down, as gravity slows the flow of milk compared with horizontal positions.

2. Can improve attachment

- Baby is able to extend their neck slightly and come to the breast “chin first,” often achieving a deeper latch.

- It encourages good alignment of the ear, shoulder and hip, which can make feeding more efficient.

3. Helpful for reflux or frequent possets/vomiting

- While it won’t treat reflux, feeding in a more upright position may reduce the amount of milk coming back up in some babies.

- Many parents find babies are more comfortable during and after feeds.

4. Supports babies with oral challenges because babies have more control over their head and jaw position, it can be useful for:

- Babies with low muscle tone.

- Babies with mild oral dysfunction.

- Some babies with a tongue-tie (although positioning alone won’t resolve tongue function issues).

- Babies who tend to lose suction or click at the breast.

5. Encourages active feeding

The upright position can promote better jaw movement and tongue function, allowing some babies to transfer milk more effectively.

6. Comfortable as babies get older

- Distractible babies.

- Quick feeds.

- Feeding while out and about.

When I often suggest trying it

* Oversupply or forceful let-down.

* Clicking or losing suction.

* Frequent coughing or spluttering at the breast.

* Reflux symptoms.

* Babies with mild muscle tone or oral coordination challenges.

* Parents with larger breasts who find cradle holds awkward.

* Babies who simply refuse more traditional positions.

Like any breastfeeding position, there isn’t one “best” hold. The best position is the one that allows effective milk transfer, a comfortable latch, and a relaxed parent and baby. The koala hold is simply another useful tool that can make a real difference for the right dyad.

This is an overachieving athlete (literally).….This is not the NORM. 🤣 MILLIE ELLIOTT

Most breastfeeding mothers pump somewhere between 30–120mL total (both breasts combined) when expressing between feeds.

If you’re replacing a feed or exclusively pumping, volumes may be higher, often around 90–150mL.

Some mothers with a full milk supply only ever pump 20–50mL after a feed because their baby is far more effective than a pump.

Pumping output is influenced by:

• How recently baby fed
• Pump quality and fl**ge fit
• Time of day
• Stress levels
• Individual breast storage capacity

Breastfed babies typically consume around 750–800mL across a 24-hour period from 1–6 months of age, that’s on average 80mls per feed.

Please don’t compare your pumping output to someone else’s bottle.

The amount you pump is measuring what the pump removed, not what your body is capable of making.

Look at how your baby is feeding, growing do they have adequate output.
