Three people can walk in and say the same four words — I'm not sleeping well — and be describing three completely different problems.

One can't get to sleep. Lies there for an hour and a half with the day still running. That's usually a wind-down problem, and it's the one that responds fastest.

One gets to sleep fine and is wide awake at three. That's rarely about sleep at all — it's the day before arriving at the one hour it can finally be heard.

One sleeps eight hours and wakes up flat. That's the hardest of the three to talk about, because on paper nothing is wrong.

They don't share a cause and they don't share a plan. Which is why the first question in the room is never "how long have you had insomnia" — it's "walk me through a bad night, start to finish."

Most people arrive with a picture in their head that's about four times too big.

An acupuncture needle is roughly two hairs wide. About forty of them fit inside the tube of a standard blood-test needle. That one fact resolves most of the nerves in the room before anything starts.

Before anything goes anywhere, we talk — usually fifteen or twenty minutes. What brought you in, how long it's been going on, what you've already tried, and what the day actually looks like. Then a first session runs somewhere between six and fifteen needles, nearly all of them below the elbows and the knees.

What you feel is a dull heaviness, or a spreading ache around the point. Sometimes nothing at all. If something is genuinely sharp, you say so and it comes straight out.

Then the room goes quiet for twenty-odd minutes. A lot of people fall asleep on the table, which is a normal outcome rather than a sign you're doing it wrong.

The whole appointment runs an hour.

Lemon balm is one of those plants that's growing in half the gardens on this coast and getting mistaken for mint.

It's traditionally used where the difficulty isn't being tired — it's that the day hasn't finished yet. Body horizontal, mind still working through tomorrow. Different problem to exhaustion, and it needs a different approach.

What it isn't is a sedative. It doesn't put anyone out, which is exactly why it tends to sit alongside the rest of the work — the last hour of the evening, the light, what the afternoon looked like — rather than standing in for it.

And the honest part: two people who both say "I can't sleep" usually don't get the same herb. One of them is wired at eleven, the other is up at three, and those are not the same situation.

One of the more useful things anyone can do is stop grading the whole month against their best week of it.

Capacity moves across a cycle. Sleep, appetite, temperature, patience, how much a normal Tuesday takes out of you — all of it. Same person, same routine, genuinely different week.

Most people describe this as being unreliable, or say they "should be able to" hold the same pace throughout. That framing costs a lot and doesn't reflect what's actually happening.

It also matters clinically. "How's your energy?" has four different honest answers depending on which week you ask, so a single snapshot is close to useless. Where you are in the month is one of the first things I want to know, because it changes what's worth looking at and what's worth leaving alone.

If the difference between your best week and your worst week has widened, or the pattern has changed from what it used to be, that's the bit worth looking at properly.

Four weeks is a long time to hold a pattern in your head, and almost nobody does it accurately — including me, about my own week.

So the second appointment often opens with "how has it been?" and closes with "yeah, alright I think", which is a shame, because the actual answer is usually more interesting than that.

What helps is small. A note on your phone, six lines, added to whenever something shifts rather than every day. What the sleep did. What the 3pm slump did. What happened after the meal you weren't sure about. Anything that surprised you.

The week that was different is the one worth having written down. It's the one that tells us what's moving and what isn't, and it's always the first thing to fall out of memory.

You don't have to be organised about it. Messy notes are fine. They're still more accurate than a recollection.

If you're comfortable in the morning and uncomfortable by mid-afternoon, that pattern is doing you a favour — it's telling you something a general "bloating" label doesn't.

A bloat that builds through the day usually has a timing problem behind it rather than a single food. Which is why the elimination diet often doesn't land: it asks what, when the more useful question was when.

Things we'd look at before touching the food list.

How fast the meal was eaten, and whether it was eaten sitting down. What the gap between meals looks like, because constant grazing never lets the gut do its clearing work. Whether stress is peaking at the same hour the bloating is. And what's actually happening at the other end — regularity tells you more than most people expect.

Sometimes it is a food. Often it's the conditions the food arrived in.

If it's been months rather than weeks, it's worth looking at properly.
