New research reinforces what women, midwives and families have been telling us for years.

Birth trauma is about more than clinical outcomes. It is also about whether a woman feels heard, respected, supported and involved in decisions about her care.

New Australian research has found that women who receive continuity of care from the same midwife, team of midwives, or doctor throughout pregnancy and birth are significantly less likely to experience birth trauma than those receiving fragmented care. Women who have continuity of midwifery also experience fewer interventions and better overall birth outcomes compared to standard fragmented care.

Professor Hannah Dahlen AM said "We must prioritise relationship based care for all childbearing women. While we have come a long way in reducing adverse physical outcomes for women and babies we are seeing increasing birth trauma, especially psychological trauma due to disrespectful care. This is having significant effects on the wellbeing of mothers, babies and families. Relationship based, respectful maternity care is the best way to reduce this."

At Australian College of Midwives, we know that relationships matter.

Knowing your midwife builds trust, supports informed decision-making and creates the continuity that helps women feel safe throughout pregnancy, birth and the postnatal period. This is why ACM continues to advocate for maternity reform that makes midwifery continuity of care accessible to every woman, regardless of where she lives or how she chooses to give birth.

Every woman needs a midwife.
Some women need a doctor.
Every woman deserves both professions working together.

Silence has consequences.

This isn't about comparing experiences—it is about reminding us what happens when we stay silent as rights, choices and professions are gradually diminished.

Across Australia, we've watched independent maternity care become increasingly difficult to provide.
We've watched midwives reported to AHPRA go through traumatic reviews of practice. We've watched the erosion of the respect for midwives. We've watched the media blame midwives, organizations diminish the role of the midwife and Home birth services disappear. Private midwifery is becoming harder to sustain. Endorsed midwives continue to face unnecessary barriers. And with every service that closes, every regulation that limits access without improving safety, and every midwife who leaves practice, women lose another choice.

This isn't just about midwives. It's about every woman having access to safe, evidence-based, woman-centred maternity care and genuine choice about who provides that care and where she gives birth.

If we don't speak up for midwives whose services are under threat today, there may be no one left to speak up when the next choice is taken away.

We don't have to agree on everything. But we do need to stand together.

Homebirth Australia has a ready made email which even helps you send it to your Member of parliament. We all have social media accounts which can support midwives, homebirth and the choices of women and birthing people. It's time for us to be standing together and speaking out, before it's too late and midwifery is eroded even further.

Because protecting independent midwifery means protecting choice for Australian women.
