15 months after breast augmentation + lift (mastopexy).

This patient underwent a combined breast augmentation and mastopexy to address both breast volume and position.

Combining an implant with a breast lift allows us to approach two different things at the same operation: the implant contributes volume, while the mastopexy reshapes and repositions the existing breast tissue and ni**le–areola complex.

6 weeks after a lower body lift with liposuction.

For the right patient, a lower body lift can address excess skin around the lower abdomen, flanks and lower back following significant weight loss or changes over time. Liposuction may also be used selectively where appropriate.

Many of you would have heard me say many times already: recovery is a marathon, not a sprint. Looking after your scars, wearing your compression garments as advised, staying active within your restrictions, and giving your body time all play an important role in the healing process.

“I don’t like this bit under my arm that sticks out over my bra!”
It’s actually a very common concern in massive weight loss patients.

For some people, the excess skin and tissue isn’t confined to the upper arm. It can extend into the underarm and side-of-chest area as well.

In those situations, a standard arm lift may not address the entire area of concern.

An extended brachioplasty continues beyond the arm and onto the side of the chest, allowing the excess skin of both regions to be treated together.

This patient is shown 3 months after surgery.

Every procedure has benefits, limitations and trade-offs, including scarring. The most appropriate treatment depends on the individual and is something we discuss carefully during consultation.

“I don’t like this bit under my arm that sticks out over my bra!”
It’s actually a very common concern in massive weight loss patients.

“Do I need a lift? Or do I need implants?”
This is one of the most common areas of confusion I see in consultation.

A breast lift and a breast augmentation are designed to do completely different things.

A lift repositions breast tissue and ni**le position higher on the chest.
An implant primarily adds volume.

An implant can influence shape to a degree.
A lift can create the appearance of fullness in certain areas.

But one does not replace the other, and many patients may benefit from a combination of both depending on their anatomy and goals.

This is why thoughtful assessment matters.
Not every concern is solved by “going bigger”, and not every patient wanting more upper fullness necessarily needs an implant.

These are the conversations we spend time properly unpacking in consultation.

“Do you do an internal bra?”
Honestly, I really dislike that term.

Social media has popularised it to the point where it can mean almost anything:
Mesh
internal sutures
supporting tissue
tightening the lower pole
implant reinforcement
or sometimes… nothing clearly defined at all.

The reality is that breast surgery is far more nuanced than a catchy phrase.

Different patients need different approaches depending on their anatomy, tissue quality, implants, previous surgery, skin laxity and long-term goals.

Sometimes supportive materials are used.
Sometimes they aren’t.
Sometimes the priority is reshaping tissue rather than “holding things up”.

That’s why good surgical planning matters more than buzzwords…
