At South Steyne Medical Centre, Chronic Disease Management consultations are bulk billed for eligible patients and can help you access structured support for conditions such as diabetes, heart disease, arthritis, asthma and other ongoing health concerns.

Working together with your healthcare team, a Chronic Disease Management Plan can help coordinate your care, set health goals and connect you with allied health services where appropriate.
