We are looking for compassionate and reliable Disability Support Workers to join our growing team. In this role, you will provide person-centred support to NDIS participants, helping them achieve their goals while promoting independence, dignity, and community participation.

Key Responsibilities:

- Provide personal care and daily living support as required.

- Assist participants with community access, appointments, and social activities.

- Support participants in developing life skills and maintaining independence.

- Complete accurate shift notes and documentation.

- Follow individual support plans and work collaboratively with participants, families, and the care team.

Service Areas:
West Melbourne, Point Cook, Werribee, Hoppers Crossing, Melton, Sunshine, St Kilda, and Sunbury.

Experience in disability support is preferred. A valid driver's licence, reliable vehicle, and relevant NDIS compliance documents are highly regarded.
