Tomorrow night (Thursday 24 September, 7:00–8:30pm AEST) I'm joining an RACGP Specific Interests panel on off-label prescribing in Australia, facilitated by A/Prof Bernard Shiu, alongside Dr Anthony Marinucci, Dr Karen Spielman, Dr Jenny Huang and A/Prof Rashmi Sharma OAM.

Most of us prescribe off-label every week, often without a second thought. But regulators are now looking closely at GLP-1 agonists, medicinal cannabis and the peptide industry, and the scrutiny won't stop there. If you were asked tomorrow to justify an off-label script, could you show the evidence you relied on, the consent you obtained and the monitoring you put in place? For many GPs, the honest answer is no.

We'll look at how to identify where an off-label prescription sits on the risk spectrum, from well-established uses with strong evidence through to novel treatments in evidence-poor areas, and how to match your consent process, documentation and monitoring to that level of risk. We'll also cover how this applies across different areas of practice and medications, and what our medico-legal responsibilities look like at each tier.

Off-label prescribing is often the best option we have, especially where approved treatments fall short. But a good clinical decision can still end in a complaint, a notification or a coronial inquiry if it isn't properly documented and defensible. Knowing where the lines are protects our patients and our registration.

A newly published protocol in npj Women's Health introduces the MELLOW study, a global collaboration between OMF’s Melbourne and Uppsala teams. The study uses an innovative chronobiology based approach, timing multi-omics sampling to menstrual phases and daily biological rhythms.

By aligning hormones, molecular data, physiology, and symptoms to real biological clocks, this research aims to improve biomarker reliability and deepen understanding of these complex conditions.

We’re excited to share a new Nature Protocols paper that our team has co-authored on ME/CFS and Long COVID (the MELLOW study).
The protocol uses a chronobiology-based design, timing multi-omics sampling to menstrual phases and daily biological rhythms to better capture how hormones, metabolism and immune function interact over time.

This kind of careful timing is essential if we want to find reliable biomarkers and deepen our understanding of these complex conditions.
