Burnout often presents subtly, particularly in individuals who have spent years masking, coping, adapting, or pushing through.

Sometimes it looks like exhaustion, shutdown, irritability, withdrawal, reduced tolerance, difficulty initiating tasks, increased sensory overwhelm, or simply no longer being able to sustain the demands of everyday life in the same way.

Sustainable participation is not built through constant pressure or productivity.
It often requires structured rest, environmental adjustments, nervous system support, reduced demands, and realistic expectations of capacity.

Sometimes, the most therapeutic plan includes protected rest.

“It feels like I'm going backwards.”

We sometimes hear this from participants & families.

The reality is that support needs are not always linear, particularly in the context of complex mental health, trauma, autism, ADHD, psychosocial disability, and nervous system overwhelm. Periods of progress can be followed by shutdown, regression, burnout, increased support needs, or reduced capacity to cope with daily demands.

This does not mean someone is failing.
Often, it reflects the reality of living with fluctuating function in complex and dynamic environments.

Bespoke OT clinicians strive to provide clear, defensible recommendations grounded in real-world functioning, not unrealistic expectations of consistency or independence.

Meaningful support begins with understanding the whole person, not just isolated moments of capacity.

Trauma does not always present in obvious ways.

Sometimes it appears as withdrawal, avoidance, emotional reactivity, shutdown, difficulty engaging, or behaviours which are easily misunderstood.

These responses are rarely intentional. More often, they reflect protective patterns shaped by a nervous system that has adapted to stress, overwhelm, or past experiences of threat.

When we understand how trauma can show up functionally, we shift from asking “what’s wrong with this person?” to “what has this person experienced, and what do they need to feel safe?”

This changes the way we support folks.
It moves us away from punishment and pressure, and toward regulation, safety, connection, and meaningful understanding.

When a child wants to eat but feels unable to, it’s not a lack of effort, it’s a nervous system response rooted in safety, sensory processing, and past experiences with food.
At its core, ARFID is not about behavior; rather, It’s about regulation.
Understanding this distinction changes how we respond, shifting from pressure to support, and from frustration to informed care.

The effort to regulate.

The effort to show up and integrate socially.

Living with psychosocial disability, trauma, and neurodivergence often means expending unseen energy for outcomes others seem to achieve more effortlessly.

For many people, particularly with psychosocial disability, trauma, autistic burnout, or ADHD, capacity fluctuates day to day.

Good assessment reflects that full picture, not just the “good” days.

If therapy doesn’t feel safe, it doesn’t work.

For many people we support, “complex history” is not just a phrase; it is lived experience and it impacts every relationship.

Our clinicians:
• prioritise regulation before goals
• collaborate instead of dictate
• work at the nervous system’s pace
• ensure choice and control

Clinical skill matters.
But relational work trumps all. Safety first. Always.

Sometimes it’s that frustrating gap between knowing what to do… and being able to start.

When that gap understood how the brain works, what’s getting in the way, things can begin to shift.
