🩵 ULTRASOUND vs. MRI vs. CT vs. LYMPHOSCINTIGRAPHY

🔎 What can each test actually show?

If you’ve ever been told, “We need imaging,” you may wonder:

Why this test? What are they looking for? And can it actually diagnose lymphedema?

The answer is: different imaging tests look at different things.



🩻 ULTRASOUND

Think: “Let’s look at what’s happening in the tissues and blood vessels.”

Ultrasound uses sound waves to create images.

It can help doctors:
✔️ Look at the soft tissues
✔️ Assess tissue thickness and changes
✔️ Examine blood flow
✔️ Look for problems such as a blood clot that could be causing swelling
✔️ Sometimes help identify lymphatic vessels or veins for treatment planning

What it doesn’t do:
A regular ultrasound does not simply prove, by itself, that swelling is lymphedema.



🧲 MRI

Think: “Let’s get a very detailed picture of the tissues.”

MRI uses magnets and radio waves to create detailed images.

It can show:
✔️ Soft tissues in great detail
✔️ Fluid within tissues
✔️ Changes in fat and other tissue structures
✔️ Fibrotic/tissue changes
✔️ Possible causes of swelling or something blocking lymph flow

Specialized MR lymphangiography (MRL) can also provide images of lymphatic anatomy and lymphatic flow patterns.

What it doesn’t do:
A standard MRI isn’t automatically a “lymphedema test.” The type of MRI and what the doctor is looking for matters.



🖥️ CT SCAN

Think: “Let’s see the body’s structures in cross-sections.”

CT uses X-rays and a computer to create detailed pictures from different angles.

It can help show:
✔️ Swelling and tissue changes
✔️ The amount and distribution of tissue changes
✔️ Enlarged lymph nodes or other abnormalities
✔️ A mass or other problem that could be blocking lymphatic drainage
✔️ Other possible causes of swelling

What it doesn’t do:
CT can show changes associated with lymphedema, but CT findings alone don’t necessarily tell you exactly why the swelling is happening.



☢️ LYMPHOSCINTIGRAPHY

Think: “Let’s actually track lymphatic movement.”

This test is different from the others.

A small amount of radioactive tracer is placed into the skin, and special cameras follow where that tracer travels through the lymphatic system.

It can help show:
✔️ Whether lymphatic transport is delayed or abnormal
✔️ How the tracer moves toward lymph nodes
✔️ Areas of dermal backflow
✔️ Differences between one side and the other
✔️ Abnormal or alternative lymphatic pathways

This test is specifically designed to evaluate lymphatic function.



🩵 SO WHICH ONE IS “THE BEST”?

There isn’t one imaging test that answers every question.

Think of them as different tools in a toolbox:

🩻 Ultrasound → What’s happening in the tissues/blood vessels?

🧲 MRI → What do the tissues and structures look like in detail?

🖥️ CT → What does the body’s anatomy look like, and could something else be causing the swelling?

☢️ Lymphoscintigraphy → How is lymphatic transport functioning?

And sometimes, more than one test may be useful, depending on the person’s symptoms and what the healthcare team needs to find out.

⭐ IMPORTANT:

Lymphedema is often diagnosed through medical history and physical examination, with imaging used when additional information is needed. No single scan should be interpreted by itself.

The right test depends on the question the healthcare professional is trying to answer.🩵🦋

🦵 Pitting vs. Non-Pitting Edema: What Does It Actually Tell Us?

When assessing swelling, one of the first things we may check is whether the tissue “pits” when we apply pressure.

But that finding is only one piece of the puzzle. 🧩

💧 What is pitting edema?

Pitting edema occurs when you press into swollen tissue and a temporary indentation remains.

✔️ Fluid can be displaced within the tissue
✔️ The tissue may feel soft or pliable
✔️ It can occur with many different causes of swelling
✔️ Early-stage lymphedema can also be pitting

🧱 What is non-pitting edema?

With non-pitting edema, pressing the tissue doesn’t leave the same lasting indentation.

✔️ The tissue may feel firmer or more dense
✔️ Chronic swelling can cause changes within the tissues
✔️ Fibrosis and tissue remodeling can make the tissue less compressible
✔️ More advanced lymphedema may become increasingly non-pitting

🚨 So… can you diagnose lymphedema by whether it pits?

No.

That’s an important distinction.

❌ Pitting does not rule out lymphedema.
❌ Non-pitting does not automatically confirm lymphedema.

Instead, pitting gives us information about how the tissue is behaving at that moment.

🔎 What else should we look at?

A proper assessment considers the bigger picture:

✔️ Where is the swelling?
✔️ When did it begin?
✔️ Is it getting better, worse, or staying the same?
✔️ What does the tissue feel like?
✔️ Are there skin or tissue changes?
✔️ Is there a history of surgery, cancer treatment, trauma, infection, or other risk factors?
✔️ Are there functional changes or heaviness in the limb?

🩵 The takeaway

Don’t diagnose the swelling by the dent.

Pitting vs. non-pitting can give us a valuable clinical clue, but understanding edema requires looking at the cause, distribution, tissue characteristics, history, and changes over time.

Because the real question isn’t:

👉 “Does it pit?”

It’s:

👉 “What is causing the swelling, and what is happening to the tissue?”

That distinction matters when we’re trying to understand and appropriately manage lymphedema and other forms of chronic swelling. 🩵
