That was the AusAlert national test. Ten seconds of siren, full volume, even on silent.

Most people jumped and moved on. But if you live with tinnitus, hyperacusis or misophonia, we know that sound may have landed very differently.

So we are asking:

🔊 Did you receive the alert?
🔊 Did you find it annoying, or worse?
🔊 Were you worried about what it might do to your ears?
🔊 Have you noticed anything since, like a tinnitus spike or ears feeling more sensitive than usual?

Nothing at all? Tell us that too. Every answer helps us understand how sudden alerts land on sensitive auditory systems.

Some reassurance. Ten seconds of alert at phone volume will not damage your hearing. What it can do is trigger your auditory alarm system. Your brain treats sudden unexpected sound as a threat, and if your system is already sensitised, the reaction can continue well after the siren stops.

There’s a film out at the moment called Tuner.

The main character has hyperacusis. He was a pianist, sound became painful, and now he tunes pianos and wears ear protection to get through an ordinary day.

The poster calls it a hidden talent.

We’ve only watched the trailer so far. It’s on Prime and Apple TV now, so we’ll watch it properly this week. We’re glad it exists. Most of the people who come to us have spent years being told they’re anxious or oversensitive, and a film does more for that in one weekend than we manage in a year.

But that tagline needs answering.

Hyperacusis isn’t a talent. The sound isn’t louder, the system is. Every auditory system has an automatic volume control. In hyperacusis it has turned itself up too far and stayed there, so ordinary sound arrives as too much. That isn’t a gift. It costs people their work and their family meals and it keeps the nervous system in fight or flight all day.

A lot of you have messaged asking what approach we actually use in the clinic, so we wanted to explain it properly.

We use Tinnitus Retraining Therapy, or TRT, for tinnitus, hyperacusis and misophonia. It always begins the same way. You are fitted with ear level sound devices, each one programmed to your own prescription.

You will also be trained in sound enrichment and sleep sound therapy, an essential component of the treatment.

The counselling side matters just as much. We take the time to explain what is happening in your brain and why sound is affecting you the way it does.

From there the plan is built around your condition, and that part really does differ. Misophonia, for instance, is not treated the same way as tinnitus or hyperacusis.

TRT was developed by Pawel and Margaret Jastreboff, and their published work shows around 80 percent of patients improve. Mini trained under them, and they still mentor her to this day, so what you get here is the real method rather than a watered down version of it.

Supermarket Dizziness !!!

"Doctor, I feel dizzy only in supermarkets!"

This is a surprisingly common complaint in dizziness clinics

Some patients report that they can walk comfortably at home, yet the moment they enter a supermarket, shopping mall, or busy store, they begin to feel unsteady, lightheaded, or as if they are swaying. Interestingly, they usually do not describe true spinning vertigo

Why does this happen?

Supermarkets are visually challenging environments. Long aisles, shelves filled with repetitive patterns, moving shoppers, shopping carts, bright lighting, and constant visual motion create an overwhelming amount of visual information.

Under normal circumstances, the brain integrates information from the vestibular system, vision, and proprioception to maintain balance. However, after vestibular disorders such as BPPV, vestibular neuritis, Ménière’s disease, vestibular migraine, or even concussion, some individuals become excessively dependent on visual cues for balance.

As a result, visually complex environments can trigger dizziness, unsteadiness, anxiety, and a feeling of disorientation. This phenomenon is often referred to as "Supermarket Dizziness" or Visual Vertigo, and it is frequently encountered in patients with Persistent Postural-Perceptual Dizziness (PPPD).

The good news is that the problem is usually not ongoing damage to the inner ear. Instead, it reflects altered sensory processing and visual dependence.

Not every dizzy patient needs more scans or more medications. Sometimes, the key is understanding how the brain is processing balance information

Yes, it's available now as a clinician's monograph: A Systems Framework for Tinnitus: Generation & Maintenance. It's written so that any reader, audiologist or not, can follow it easily, while still carrying the depth a specialist clinician needs.

It is designed to develop the clinical reasoning that chronic conditions like tinnitus demand. Clear thinking has to come before any diagnosis, and building that reasoning is what this book is for.

In the book's own words, on what it is and is not:

"It is deliberately not a treatment manual. It will point clearly to where treatment should be aimed, to which system and in which patient, but the detailed protocols of sound therapy, cognitive therapy and the rest belong to their own dedicated texts. The framework's job is to tell you what to aim at and why."
