πββοΈπ¦΅ SHIN SPLINTS: What actually works?
There isn't one proven βbest treatmentβ for shin splints. A new 2026 systematic review investigating treatment for medial tibial stress syndrome (MTSS) found only:
π 3 randomised controlled trials (130 runners) = No clearly superior treatment
So should we use orthotics? Strengthening? Gait retraining? Shockwave? Different shoes?
Sometimes. But probably not the same treatment for every runner.
This fits closely with Professor Christian Barton's approach to running rehabilitation recently discussed in the Australian Physiotherapy Association's InMotion.
His central principle: Every runner is unique and running injuries are multifactorial.
Rather than following a recipe, rehabilitation should consider:
π Training load
𦡠Physical capacity
πΉ Running gait where relevant
π Footwear/orthoses where relevant
ποΈ Exercise prescription
π A structured return-to-run plan
π§ Education and confidence
What does the MTSS research tell us?
π Orthotics: may reduce short-term pain in some runners, but benefits haven't been shown to persist.
ποΈ Exercise: important for restoring capacity, but no particular programme has been proven to βcureβ shin splints.
π₯ Shockwave: evidence is mixed and does not support treating it as a guaranteed solution.
π Gait retraining: useful when assessment identifies something relevant β not because every runner needs their foot strike or cadence changed.
The key question probably isn't: βWhat's the best treatment for shin splints?β
It's: βWhy has this runner's current load exceeded their capacity β and what do we need to change so they can tolerate running again?β
