Why did we ask to see you walk, when it is your back that hurts?

Because the pelvis sits in the middle of a chain, and it takes whatever the ground gives it. Every step, the force from your foot travels up through the ankle, the knee and the hip before it reaches the pelvis and the spine.

If something further down is not doing its share — a stiff big toe, an ankle that has never fully recovered from a rolled ligament in your twenties, a hip that has quietly lost range — the pelvis ends up absorbing what the leg did not.
Which is why treating only the sore spot can work brilliantly for a fortnight, and then stop working.

Caitlin spent Saturday with Professor Rob Newton — one of the leading voices in exercise oncology, and the reason a lot of Australian cancer care now treats exercise as part of the treatment plan rather than something to get to later. 🧡

GLP-1 medications are a bullet train to a result. Fast. Effective for a lot of people. That's not the question.

The question is what's being built alongside them — because that's what determines whether the result holds when the dose steps down or stops.

A recent trial for one medication in this class gave us a look. In the placebo group, patients held less than half of their loss within a year, and more than 60% needed to go back on something.*

That's one drug, and different medications in the class may look different. But it's enough to name the gap: if the adjunct lifestyle work isn't happening during the medication phase, stopping isn't a plateau. It's a rebound.

Our role as exercise physiologists isn't to prescribe or discourage — that's a doctor's call. Our role is to make sure the muscle, bone and habits are being built underneath, so when the medication stops (or the dose steps down) the result actually holds.

That means we track more than the scales. Grip strength. Leg strength. Blood pressure. Body composition. Because the scales will plateau — they always do — and if that's the only metric, a plateau reads as failure and people quit.

Weight loss becomes weight management. The goal isn't the number you hit. It's the number you hold.
