A positive syphilis screening result in pregnancy can trigger panic, suspicion and some very uncomfortable conversations.

But a screening result is not always a diagnosis. False positives do happen, particularly in women with no obvious risk factors, which is exactly why confirmatory testing is so important.

Before changing the locks or launching a private investigation, wait for the full results. Sometimes the only lasting consequence is a very awkward story you laugh about years later.

Not that long ago, if a woman had an ovarian cyst, medicine was often very comfortable removing the whole o***y.

Sometimes through a large incision. Sometimes with the casual explanation of, “you’ve got two anyway.”

Now imagine applying that same logic to men with testicles.

The problem is, ovaries are not spare parts. They are complex endocrine organs, and over time, research has forced us to confront what many women were never properly told: removing an o***y before menopause may have long term consequences for hormones, cardiovascular health and cognition.

That does not mean an o***y should never be removed. Sometimes it is absolutely the safest and most appropriate decision. But it should never be treated as irrelevant.

Medicine is, in many ways, the story of slowly discovering that the body is smarter than we are.

Women deserved to know that sooner.

One of the most awkward compliments a male gynaecologist can receive is, “You’re much gentler than my female doctor.”

Male gynaecologists have no personal reference point for periods, childbirth, endometriosis, speculum examinations or cervical screening. So we tend to approach these examinations very carefully, because the last thing we want is for a patient to feel rushed, dismissed or hurt.

Of course, the best doctors, male or female, are gentle because they listen, communicate and pay attention to the person in front of them.

And when it comes to women’s health, that matters more than anything.

Testosterone in perimenopause is often misunderstood.

It is not a magic fix, and it is not simply a “male hormone”. Women produce testosterone too, and receptors are found throughout the body, including the brain, bones, muscles and reproductive tract.

For some women, especially when low desire is tied to fatigue, low motivation, brain fog and feeling unlike themselves, testosterone may be helpful when used appropriately.

It does not necessarily create desire out of nowhere. Sometimes it helps remove some of the things standing in the way.

For many parents, NIPT begins as the test that reveals their baby’s gender. But its real purpose is far more important.

NIPT is a powerful screening tool that estimates the chance of certain chromosomal conditions, such as Down syndrome. It is not a diagnosis, and understanding that difference is essential.

A high risk result doesn’t always mean a baby is affected, but it may lead to further testing and difficult decisions. That’s why these conversations should never be reduced to numbers alone.

Good obstetric care isn’t just about ordering tests. It’s about helping families understand what the results mean, what they don’t mean, and supporting them through every step that follows.

It is much easier to be a nice doctor than a good doctor.

A nice doctor tells you what you want to hear. A good doctor has the courage to tell you what you need to hear.

The hardest decisions in medicine are not always seen by the patient. The worry, the responsibility, the sleepless nights, the weight of trying to make the right call.

Kindness matters. Compassion matters. But kindness without courage can become people pleasing.

Real care sometimes means being misunderstood in the moment, because someone is relying on you to do what is right.

The best lessons are rarely on the slides.

Medical conferences can look like lanyards, lectures and suspiciously average finger food.

But the real value is often in the conversations between sessions. The shared experience, the difficult cases, the quiet advice and the reminder that medicine is bigger than any one hospital.

Sometimes, that is where the most useful learning happens.

Some things should simply not be said during labour.

Nobody expects partners to be perfect. Most are nervous, overwhelmed and trying their best. But in labour, the most helpful support is often very simple.

Stay calm. Offer encouragement. Help with water, timing, advocacy and comfort. And most importantly, listen to what she actually needs in that moment.

Because long after birth, mothers may not remember every detail. But they often remember whether they felt safe, supported and heard.

A birth plan is important, but birth does not always follow the plan.

It can be a helpful way to communicate your preferences, your values and the experience you are hoping for. Pain relief, delayed cord clamping, skin-to-skin, music, lighting and cultural requests all matter.

But labour can change quickly, and sometimes the safest birth is not the one that was originally imagined.

A good birth plan leaves room for both autonomy and safety.

Healthy mother. Healthy baby. Always the North Star.
