Performing Breast cancer surgery my goals are to firstly remove all of the cancer and secondly to do it in a way that preserves femininity, reduces scars and deformity and maintains shape and symmetry. Sometimes you can get a very satisfying outcome on all of these goals utilising oncoplastic surgery techniques as I did in this patient. Her breast is a little swollen post op due to lymphoedema following radiotherapy, which settled later on in the recovery.

This is a rare scenario Inoperated on the other day. It’s in interesting case so I thought I’d share. This unfortunate person had a bad attack of gall bladder inflammation leading to gall bladder fusion to the intestine sitting next to it and the. The large stones sitting within it perforated through into this intestines, but the stones were so big (size of egg) they couldn’t get through the intestine causing a bowel obstruction and the person to be very sick. I delivered this stone from the bowel through an open abdomen and then closed everything back up allowing the system to reset itself and heal up. We don’t see this very often , but when we do it’s always a good story.

This lovely 63 yo woman has had implants for 30 years and of course they were ruptured and misshapen by now. Also notice the typical waterfall deformity which is obvious on the lateral view, where the implants are still in original location helped by capsular contracture but the breast is sliding off the implant and chest wall and is very ptotic/saggy. So we have an implant issue and a breast issue. She also had a previous breast lift in the past complicating the picture. We needed to find low maintenance, aesthetic, elegant look that would work for her moving forward. In these cases I feel an Explant with breast lift with contouring fat graft to give some cleavage and upper pole volume is a good long term solution and something our patient was very happy with.

This young woman unfortunately developed breast cancer. My goals in her surgery apart from saving her life and removing cancer completely is to make sure that she has a good quality of life once we have beaten the cancer. To achieve that in her case I wanted to maintain the good shape that she had in her breast pre operatively, minimising distortion and deformity and asymmetry, whilst not compromising on the oncological goals of the operation. I’m pretty happy with the outcome. What do you think?

This 32 yo woman had 5 kids and carried a lot of extra weight at the end of that journey. She managed to lose it with hard work and time, but that left her true figure hiding inside the old suit of large excess of skin. The FDL LipoAbdominoplasty was a procedure that suited her needs. The additional scars are reasonably well hidden and are acceptable trade off for her to regain the shape and contour of lower and upper abdomen and hips. It’s about quality of life, which is hopefully significantly improved post this surgery.

Inner and outer thigh contouring particularly under general anaesthesia can be quite challenging to get right. It is super easy to overresect some areas creating unevenness and deformity, which is even more difficult to fix after. The reason for this is that tissue distribution is very different when you are standing upright versus when you are lying on your back or front and everything is splayed out. I prefer to be on the slightly conservative side in these procedures, as I always tell my patients I can take more out but very hard to put it back in. So be careful who you choose to do these procedures which are deceptively straight forward, but actually not.
