Artificial Intelligence in Osteopathic Medical Education: Opportunity, Risk, and the Call for Intentional Integration; A Letter to the Editor - Artificial intelligence (AI) tools, particularly large language models and generative AI platforms, are rapidly reshaping the preclinical medical education environment. Medical students increasingly use these tools for content review, board preparation, and clinical reasoning exercises, while faculty are beginning to explore AI-assisted case generation, adaptive feedback, and automated assessment. This letter to the editor examines the emerging evidence on AI in medical education through the lens of osteopathic medical training, where clinical reasoning depends not only on algorithmic knowledge but on the integration of structural examination, palpatory findings, and patient-centered care.

Thoracic Manipulation versus Myofascial Release on Pain, Function, and Proprioception in Patients with Shoulder Impingement Syndrome: A Randomized Controlled Trial. - To investigate the adding effects of thoracic manipulation (TM) versus myofascial release using instrument-assisted soft tissue mobilization (IASTM) to scapular stabilization exercises (SSEs), on pain, range of motion (ROM), proprioception, pain pressure threshold (PPT), and disability in patients with shoulder impingement syndrome (SIS).

Patient Safety and Regulatory Gaps in Manipulative Therapy Practices in Indian Physiotherapy- A commentary - Musculoskeletal disorders (MSDs) are the primary cause of disability in many parts of the world, and physiotherapy is commonly included within evidence-informed management strategies for those conditions. However, in India, concerns have emerged regarding the use of professional titles such as "chiropractor" and “osteopath” in the absence of clear statutory regulation, standardized educational pathways, competency-based training frameworks, and formal licensing mechanisms. Some practitioners perform high-velocity manipulative procedures following short-term or non-standardized training pathways, which may increase concerns regarding the recognition of contraindications, clinical decision-making, and safe practice.

Artificial intelligence in osteopathy education: A critical analysis of educators' and researchers' perspectives - In response to the recent article by Mhadhbi et al.. [1], “Osteopathy Educators' and Researchers' Perspectives on Artificial Intelligence in Academia: A Cross-Sectional Study”, presents an intriguing exploration of the integration of artificial intelligence (AI) in osteopathy education. The study offers valuable insights into how AI is perceived by educators and researchers, and its potential to shape the future of academic practices. This topic is highly relevant in the current educational climate, where digital transformation and technological advancements are crucial.

Rethinking causation and mechanisms in osteopathy and physical intervention trial design: the positivist-realist continuum - There has been renewed interest in mechanistic theory within osteopathy and the associated musculoskeletal professions, particularly regarding the role mechanisms play in osteopathic, manual and other physical interventions. Evidence-Based Medicine which commits to evidential pluralism (explicit incorporation of mechanistic evidence to inform trial design, through the selection of more meaningful controls and measuring more relevant outcomes) is known as Evidence-Based Medicine Plus (EBM+).This position paper introduces the concept of EBM+ to the osteopathy and MSK research community, and proposes the evolution of a theory of mechanisms that borrows from both positivist and realist traditions to better account for the mechanistic effects of physical interventions that are described in trials as specific, non-specific and contextual effects, and which have a relevance for clinical practice and education.

Reframing the association between adverse childhood experiences and autoimmune rheumatic disease: Moving from observed links to underlying mechanisms - The central idea we wish to emphasize is that the relationship between ACEs and ARD should not be understood merely as a statistical association, but as a multilayered biopsychosocial process operating across the life course. Early adversity may alter neuroendocrine regulation, immune calibration, inflammatory reactivity, health behaviors, and access to care, thereby creating a long arc of susceptibility rather than a simple linear cause-effect chain [3]. This issue is increasingly important in an era marked by chronic stress exposure, mental health burden, social inequality, and expanding recognition of trauma as a determinant of long-term physical illness [4].

Fatigue in long COVID: An exploratory pragmatic randomized trial of osteopathic care plus physical therapy versus physical therapy alone - This study explored whether osteopathic care (OC) combined with physical therapy (PT) could be more effective than PT alone in reducing fatigue and functional limitations in adults with long COVID.

Perceptions of osteopaths with specific undergraduate psychologically informed osteopathic training regarding its usefulness in clinical practice: a mixed-methods study with implications for education - Musculoskeletal pain is a leading cause of disability, with psychosocial factors influencing its onset and persistence. Biopsychosocial approaches are recommended for pain management, but their integration into osteopathic practice is limited. This study explores the perceptions of osteopaths trained in a psychologically informed osteopathic practice for pain management at the Italian Academy of Osteopathic Medicine. Using surveys and focus groups, it assesses the usefulness and clinical applicability of the training.

Response to: “From documentation to judgment: What peer-supported audit really changes in IFOMPT implementation” - We thank Wahyudin and colleagues for their commentary on our study [1]. Their letter raises important questions about the mechanisms through which peer-supported audit may produce practice change [2], and we welcome the opportunity to reflect further on our findings in light of their observations.

Professionalization and Identity Construction of French Osteopathy: Toward a Hybrid Model - This professional commentary analyzes the professionalization and identity construction of osteopathy in France against a backdrop of partial legal recognition and fragmented regulation. Drawing on a cross-disciplinary theoretical framework (Freidson; Abbott; Dubar; Wittorski; Larose), this commentary provides a multidimensional interpretation of the tensions between individual autonomy and collective structuring. In comparison with the British, American, Australian, New Zealand, Swiss, Belgian, and Italian trajectories, French osteopathy emerges as a hybrid model.
