A chemical pregnancy is a very early pregnancy loss — and despite the name, it is still a pregnancy, and the loss can feel very real.

It occurs when hCG rises enough to produce a positive pregnancy test, but the pregnancy stops developing before it can be seen on ultrasound.

You may notice a positive test followed by a negative test, bleeding around or after your expected period, or sometimes no obvious signs at all. Chemical pregnancies are commonly associated with chromosomal abnormalities that prevent the pregnancy from developing normally.

Experiencing one chemical pregnancy does not necessarily mean there is a fertility problem. However, if you’ve experienced recurrent early pregnancy losses, further investigation may be appropriate.

Why am I not falling pregnant? It’s one of the most common — and often most difficult — questions I hear.

There isn’t always one simple answer. Ovulation, egg quality, s***m health, fallopian tubes, uterine factors, age and timing can all play a role, and sometimes initial testing doesn’t reveal an obvious cause.

What can an ultrasound actually tell us about your fertility? More than you might realise — but it’s only one piece of the fertility puzzle.

A fertility ultrasound allows me to assess your ovaries and uterus, including:

• The number and appearance of ovarian follicles
• Fibroids and ovarian cysts
• Some polyps and changes within the uterine cavity
• The endometrial lining
• The overall anatomy of the uterus and ovaries

However, an ultrasound can’t assess your entire fertility picture. Your age, medical history, hormone levels, ovulation, fallopian tubes and your partner’s fertility may also need to be considered.

An ectopic pregnancy can be frightening, and one of the questions I’m often asked afterwards is: “Will I be able to conceive again?”

For many women, the answer is yes. Having an ectopic pregnancy does not automatically mean you will experience fertility problems in the future.

Your individual chances of conceiving will depend on several factors, including the health of your fallopian tubes, whether one was damaged or removed, the cause of the ectopic pregnancy and your overall fertility health. Even with one healthy fallopian tube, natural conception can still be possible.

It’s also important to know that having had an ectopic pregnancy increases the risk of experiencing another. This is why I recommend speaking with your doctor when you’re ready to try again and arranging early monitoring when you next become pregnant.

IVF or ICSI — what’s the difference, and why might one be recommended over the other?

While both are used in fertility treatment, the way fertilisation occurs is different. With IVF, eggs and s***m are placed together in the laboratory and fertilisation is allowed to occur. With ICSI, a single s***m is selected and injected directly into an egg.

Which approach is appropriate depends on your individual circumstances, including s***m factors, previous fertilisation outcomes and your treatment history.
