A common misconception is that the ni**le is routinely removed during breast reduction surgery. In most cases the ni**le remains attached to a platform known as a ‘pedicle’ of breast tissue and is repositioned while preserving its blood supply and sensory nerve supply. Occasionally a ni**le graft is performed in patients who have anatomical features that place them at high risk for ni**le necrosis.
