✨We’re Hiring – Support Coordinator ✨

Are you passionate about empowering people living with disability to achieve their goals and live more independently? We’re looking for a dedicated and experienced Support Coordinator to join the team at Circle of Hope 💜

In this role, you’ll support participants through:
✔️ NDIS intake, referrals & plan reassessments
✔️ Connecting participants with Allied Health & community supports
✔️ Capacity building & promoting independence
✔️ Monitoring quality, safe and person-centred supports
✔️ Building strong relationships with participants, families & stakeholders

We’re looking for someone with strong NDIS knowledge, excellent communication skills and a genuine passion for making a difference in people’s lives.
