Why You Wake Up With a Sore Back β And What Your Sleeping Position Has to Do With It
Ever gone to bed feeling fine, only to wake up the next morning with a stiff, aching or painful lower back?
You're not alone.
Morning back pain is extremely common, but it doesn't necessarily mean you've "damaged" your spine while sleeping. In many cases, the way you sleep, the support provided by your mattress and pillows, how long you remain in one position, and the quality of your sleep can all influence how your back feels when you wake.
Importantly, there isn't one perfect sleeping position for everyone. The goal is to find a position that is comfortable, allows you to sleep well and doesn't consistently aggravate your symptoms.
ποΈ Why can your back hurt more in the morning?
During sleep, you can spend several hours relatively inactive and in a limited number of positions.
For someone already experiencing back pain or stiffness, prolonged positioning may contribute to increased symptoms when getting up.
But there's another important factor: sleep and pain influence each other.
Research consistently shows an association between poor sleep and low back pain. A systematic review and meta-analysis of 21 studies found that sleep disturbance was associated with back pain, while more recent research suggests that poor sleep may also be associated with greater pain intensity and a lower likelihood of recovery.
This relationship appears to work in both directions:
Poor sleep β increased pain sensitivity β poorer sleep
So sometimes the problem isn't simply your mattress or sleeping position.
π€ Is your sleeping position causing your back pain?
It can contribute β but it's probably not the whole story.
A 2025 systematic review specifically examining sleep posture and low back pain found that sleeping position may influence symptoms, with supportive side-lying and supine positions generally appearing more favourable than prone sleeping. However, the evidence base was limited, consisting of only six observational studies.
That means we shouldn't tell everyone:
"Sleep on your back and your pain will disappear."
Bodies are different.
Some people feel better on their side. Others prefer their back. Some people can only sleep comfortably on their stomach.
Comfort and symptom response matter.
π Sleeping on your side
Side sleeping is extremely common.
If you're a side sleeper with morning back pain, one thing worth experimenting with is placing a pillow between your knees.
This can make the position feel more comfortable and may reduce unwanted rotation through the pelvis and trunk.
You can also experiment with your pillow height.
Your head should be supported rather than dropping significantly toward the mattress.
Try this:
Side + supportive pillow between knees + comfortable head/neck position.
Don't obsess over achieving a perfectly straight spine.
Your body doesn't need to be perfectly symmetrical all night.
π΄ Sleeping on your back
For some people, lying on the back can be comfortable because it provides relatively even support.
If lying flat makes your lower back uncomfortable, try placing a pillow underneath your knees.
This can make the position feel more comfortable by changing the position of your hips and lower back.
Again, this isn't a universal prescription.
If it makes you feel worse, don't force it.
π¬ What about sleeping on your stomach?
This is where things can become interesting.
Stomach sleeping can place the neck into rotation because you need to turn your head to breathe.
Depending on the individual, it may also place the lumbar spine into a position that isn't comfortable for prolonged periods.
The 2025 systematic review found an association between prone sleeping and greater low-back-pain risk, although the evidence was limited and doesn't establish that stomach sleeping causes back pain.
If you're a stomach sleeper and waking with back pain, it may be worth experimenting with another position.
But don't suddenly force yourself into a position that prevents you from sleeping.
A comfortable night's sleep is important too.
ποΈ And what about your mattress?
This is one of the most overlooked factors.
You don't necessarily need the hardest mattress possible to protect your back.
In fact, the evidence doesn't support the idea that "harder is always better."
A systematic review of controlled trials found evidence favouring medium-firm mattresses for comfort, sleep quality and spinal alignment.
A 2026 review of 47 studies similarly concluded that the evidence most consistently favoured medium-firm mattresses over very soft or very firm surfaces for adults experiencing nonspecific low back pain or morning stiffness.
The takeaway?
If your mattress is:
Too soft β your body may sink excessively.
Too firm β pressure points may develop and you may struggle to find a comfortable position.
Medium-firm β may provide a useful balance between support and pressure distribution.
But again, there is no universally perfect mattress.
π It's not just about your sleeping position
Here's the part many people overlook.
If you're waking up sore every morning, don't immediately blame your mattress.
Consider the bigger picture:

Poor sleep can influence pain sensitivity and recovery.
πΉ Activity levels
If you're sitting for most of the day and then expecting your back to tolerate normal movement, your overall physical conditioning may matter more than your sleeping position.
πΉ Previous injuries
Previous episodes of back pain can increase the likelihood of recurrent symptoms.
πΉ Stress
Stress and poor sleep can interact with pain processing and may contribute to persistent symptoms.
πΉ Physical conditioning
A strong, conditioned body generally has greater capacity to tolerate everyday loads.
πΉ Your expectations about pain
Pain doesn't always equal tissue damage.
Australian clinical guidance for low back pain recommends education, reassurance, healthy sleep habits, maintaining activity and individualised exercise as important components of management.
πΆββοΈ What should you do when you wake up sore?
Don't necessarily jump straight out of bed.
Try a few gentle movements first.
For example:
1οΈβ£ Roll onto your side
2οΈβ£ Sit up gradually
3οΈβ£ Take a few comfortable movements through your hips and spine
4οΈβ£ Get up and walk around
5οΈβ£ Gradually return to your normal activities
The Australian Commission on Safety and Quality in Health Care recommends maintaining activity and progressively increasing activity according to individual tolerance rather than avoiding movement altogether.
π¨ When morning back pain needs further assessment
Morning stiffness is common, but persistent or severe symptoms shouldn't simply be blamed on your sleeping position.
Seek professional assessment if your back pain is:
persistent or progressively worsening
associated with significant leg pain
associated with numbness or weakness
interfering substantially with sleep or daily activities
accompanied by unexplained weight loss, fever or feeling systemically unwell
associated with changes to bladder or bowel function
following significant trauma.
These symptoms can require assessment for causes beyond routine mechanical low back pain.
π§ The bottom line
Your sleeping position may influence your back pain β but it probably isn't the whole explanation.
The latest evidence suggests that supportive sleeping positions, appropriate mattress characteristics and good sleep quality can all play a role. But back pain is multifactorial, and your activity levels, physical conditioning, previous injuries, stress, sleep and nervous-system sensitivity can also contribute.
So before buying a new mattress or forcing yourself to sleep in a particular position, consider the bigger picture.
Your back doesn't necessarily need more rest.
Sometimes it needs better movement, better conditioning and better recovery.
If you're consistently waking with back pain, a proper assessment can help identify the factors contributing to your symptoms and determine whether changes to sleep position, exercise, mobility, load management or other treatments may be appropriate.
