At Take a Breath Disability Support Services, we are a family-led, lived experience provider supporting people with disability to navigate the NDIS with confidence, clarity and choice.

Support Coordination helps participants understand, implement and make the most of their NDIS plans. It’s about turning funding into real, meaningful supports that align with your goals and everyday life.

🤝 What Does a Support Coordinator Do?

A Support Coordinator works alongside you to:
• Understand your NDIS plan and funding
• Build a strong, reliable support team
• Connect you with suitable service providers
• Coordinate and monitor supports
• Assist with problem-solving and changes as needs arise
• Prepare for Plan Reviews and Plan Reassessments,Advocates for you and ensures supports stay on track

We focus on Choice, Control and Capacity Building, supporting you to feel informed, empowered and supported,not overwhelmed.

🌿 We’re Growing Our Team – Support Coordinator Wanted 🌿

Take a Breath Disability Support Services is expanding, and we’re looking for an experienced Support Coordinator to join our team in Yeppoon.

We are a family-led, lived-experience provider, committed to delivering person-centred, ethical and meaningful supports to NDIS participants and their families.

💼 About the Role

As a Support Coordinator, you will:
• Support participants to understand and implement their NDIS plans
• Build and maintain strong support networks and service teams
• Coordinate supports in line with participant goals, choice and control
• Monitor funding, outcomes and plan utilisation
• Liaise with families, providers and the NDIA
• Prepare participants for plan reviews and respond to changing needs

✅ What We’re Looking For

• Essential: Experience and strong knowledge of the NDIS
• Excellent communication and organisational skills
• Ability to work independently and collaboratively
• A values-based approach aligned with person-centred practice
• Local to, or willing to work in, Yeppoon
