Drooling of Saliva

Major Oral & Maxillofacial Surgery -এর পর Drooling : Clinical Interpretation ও Management

Oral cancer-এর neuromuscular/functional অংশ resection-এর পর drooling অনেক সময় প্রকৃত hypersalivation-এর কারণে হয় না। বরং altered oral anatomy, neuromuscular dysfunction, sensory impairment এবং impaired swallowing coordination (dysphagia)-এর কারণে স্বাভাবিক পরিমাণের saliva মুখের মধ্যে ধরে রাখা ও কার্যকরভাবে swallow করা সম্ভব হয় না।Evidence-based management-এর ক্ষেত্রে একটি multidisciplinary team approach অত্যন্ত গুরুত্বপূর্ণ। এতে behavioral, physical, pharmacological এবং surgical interventions সমন্বিতভাবে ব্যবহার করা হয়।

কেন Drooling হয়?

Resection → altered anatomy + reduced tongue mobility/strength + sensory impairment + impaired oral competence → reduced spontaneous swallowing → saliva pooling → anterior leakage/drooling

সম্ভাব্য কারণগুলো:

Reduced tongue mobility/strength

Reduced tongue–palate contact

Impaired lip seal/oral competence

Oral-phase dysphagia

Reduced spontaneous swallowing frequency

Loss of oral/tongue sensation

Facial nerve dysfunction

Hypoglossal nerve dysfunction

Altered floor-of-mouth anatomy

Mandibular resection/reconstruction

Postoperative radiotherapy-induced fibrosis

Practical Evidence-Based Management

প্রথমে Mechanism নির্ণয় করতে হবে:

প্রথম প্রশ্ন:

> রোগীর কি সত্যিই excessive salivary production হচ্ছে, নাকি normal amount of saliva manage/swallow করতে পারছেন না?

এটি অত্যন্ত গুরুত্বপূর্ণ।

Assessment করতে হবে:

A. Salivary production

সত্যিই hypersalivation আছে কি না

B. Oral competence

Lip competence

Tongue mobility

Buccal control

Oral cavity anatomy

C. Swallowing function

Spontaneous swallowing frequency

Oral bolus control

Oral transit

Pharyngeal clearance

Aspiration risk

D. Sensory function

Tongue/oral mucosal sensation

Swallowing Rehabilitation — First-Line:

Speech-Language Pathologist (SLP) দ্বারা swallowing assessment গুরুত্বপূর্ণ।

প্রয়োজনে:

Tongue-strengthening exercises

Lingual range-of-motion exercises

Oral bolus-control training

Sensory stimulation

Lip-closure/oral competence training

Swallow initiation training

Airway-protection strategies

Appropriate head and neck postural strategies

Significant dysphagia বা aspiration সন্দেহ হলে:

FEES (Fiberoptic Endoscopic Evaluation of Swallowing)

VFSS/MBS (Videofluoroscopic Swallow Study / Modified Barium Swallow)

বিবেচনা করা উচিত।

Palatal Augmentation Prosthesis (PAP):

বিশেষ করে partial/total glossectomy বা significant tongue resection-এর পর Palatal Augmentation Prosthesis (PAP) গুরুত্বপূর্ণ হতে পারে।

PAP palatal vault-এর effective height কমিয়ে residual tongue-এর সঙ্গে tongue–palate contact বাড়াতে সাহায্য করে।

ফলে:

Improved tongue–palate contact → improved bolus propulsion → improved oral-phase swallowing → reduced saliva pooling/drooling

অতএব, major tongue resection-এর ক্ষেত্রে:

PAP + Swallowing Rehabilitation

শুধু salivary secretion কমানোর চেয়ে বেশি physiologically rational approach হতে পারে।

Pharmacological Management:

যদি rehabilitation সত্ত্বেও clinically significant sialorrhea থাকে এবং সত্যিই excessive salivary secretion contributing factor হয়, তখন anticholinergic therapy বিবেচনা করা যেতে পারে।

*Glycopyrrolate / Glycopyrronium

সম্ভাব্য adverse effects:

Xerostomia

Thick/tenacious secretions

Constipation

Urinary retention

Blurred vision

Tachycardia

Cognitive adverse effects

বিশেষ করে post-radiotherapy patient-এ excessive salivary suppression করলে saliva অত্যধিক thick হয়ে swallowing এবং secretion clearance আরও কঠিন হতে পারে।

তাই dose patient-specific হতে হবে।

*Atropine / Scopolamine

Selected cases-এ:

Sublingual Atropine

Scopolamine (Hyoscine)

ব্যবহার করা যেতে পারে।

তবে systemic anticholinergic adverse effects এবং thick secretions-এর বিষয়টি বিবেচনা করতে হবে।

৬. Botulinum Toxin

Persistent/refractory sialorrhea-তে:

Ultrasound-guided Botulinum toxin injection

Parotid ± Submandibular glands-এ দেওয়া যেতে পারে।

Mechanism:

Botulinum toxin → cholinergic neurotransmission inhibition → reduced salivary gland secretion

Postoperative altered anatomy-এর ক্ষেত্রে ultrasound guidance বিশেষভাবে উপকারী।

তবে significant dysphagia থাকা রোগীর ক্ষেত্রে excessive salivary reduction এড়িয়ে চলতে হবে।

Salivary Gland Radiotherapy:

Salivary gland radiotherapy routine first-line treatment নয়।

বিশেষ করে যেসব patient-এর:

Previous radiotherapy

Xerostomia

Thick/tenacious saliva

Severe dysphagia

Dental complications

আছে, তাদের ক্ষেত্রে additional salivary suppression ক্ষতিকর হতে পারে।

তাই এটি কেবল highly selected refractory/palliative cases-এ multidisciplinary decision-এর মাধ্যমে বিবেচনা করা উচিত।

৮. Surgical Management

Refractory cases-এ options হতে পারে:

Submandibular gland excision

Salivary duct ligation

Duct rerouting

তবে previous oral cancer surgery, neck dissection, reconstruction, fibrosis, altered vascular anatomy এবং wound-healing/fistula risk থাকার কারণে এগুলো সাধারণত last-line options।

সবচেয়ে গুরুত্বপূর্ণ Clinical Principle

> Postoperative drooling ≠ necessarily hypersalivation.

অনেক oral cancer resection patient-এর ক্ষেত্রে মূল সমস্যা হলো:

Altered anatomy + neuromuscular dysfunction + sensory impairment → impaired oral competence/swallowing → saliva pooling → anterior drooling

তাই practical approach হওয়া উচিত:

“saliva কমানো” নয়—প্রথম লক্ষ্য হলো “saliva effectively contain এবং swallow করতে পারার ক্ষমতা পুনরুদ্ধার করা।”

Organizations like the World Health Organization (WHO) and contemporary researchers define these conditions as polymicrobial infections. They are driven by a shift from healthy, commensal oral bacteria to highly destructive, anaerobic dysbiotic biofilms.

1. Odontogenic Infections (Pulpal & Deep space)

Odontogenic infections originate within the tooth pulp or the immediate surrounding periapical tissues. They are primarily caused by dental caries progressing deep into the root canal structure. Global clinical consensus reveals that while initial invaders are aerobic, advanced deep space infections are dominated by obligate anaerobes.

*Primary Facultative Aerobes: Viridans group streptococci (such as Streptococcus mitis, S. sanguinis, and S. anginosus) initiate tissue entry by lowering oxygen levels.

*Dominant Strict Anaerobes: Prevotella species (e.g., P. intermedia), Fusobacterium nucleatum, and Peptostreptococcus species multiply rapidly once anoxia is achieved.

Pathology: These mixed bacteria utilize synergistic mechanisms to breach anatomical barriers, potentially resulting in cellulitis or deep facial space abscesses.

2. Periodontal Infections (Gingiva & Bone Support)

Periodontal diseases are inflammatory conditions that destroy the attachment fibers and alveolar bone holding teeth in place. According to classic WHO Institutional Data and Socransky’s color-coded microbial complexes, subgingival plaque shifts from harmless Gram-positive cocci to specialized Gram-negative pathogens.

*The Red Complex (High Virulence): Porphyromonas gingivalis, Tannerella forsythia, and Treponema denticola form a highly destructive group strongly tied to severe bone loss.

*Keystone Pathogen Hypothesis: Research names P. gingivalis as a "keystone" microbe because it subverts the host's immune system, turning a mild biofilm into a highly aggressive infection.

**BANGLADESH EVIDENCE — ORAL & OROFACIAL ANTIMICROBIAL RESISTANCE**

*A 2008 Bangladesh study of 183 patients undergoing oral and maxillofacial surgery found culture-positive infection in 44.8%; Streptococcus spp. were predominant, followed by S. aureus, Pseudomonas, E. coli and Proteus. Viridans streptococci represented one-third of the streptococcal isolates.

*More importantly, a 2022 study from Mymensingh Medical College Hospital evaluated 131 children with oral/periodontal disease and tested isolates against 12 antibiotics. S. aureus and S. salivarius were frequent isolates, and multidrug resistance was very common in the studied population; resistance-associated genes including blaTEM, cfxA and mecA were also investigated.

27-year-old man presented to Dept.of casualty after he was involved in a motor vehicle collision. The attendace report reveals periods of transient loss of consciousness followed by a lucid interval . Which of the following is the most likely diagnosis?

Epidural hematoma

Lucid Interval:

Lucid interval হলো traumatic head injury-এর পর এমন একটি সাময়িক সময়, যখন রোগী আবার স্বাভাবিক consciousness ও মানসিক স্বচ্ছতা ফিরে পান—কিন্তু এর পর হঠাৎ দ্রুত neurological deterioration শুরু হতে পারে।

এই phenomenon বিশেষভাবে epidural hematoma (EDH)-এর সঙ্গে classically associated। EDH হলো skull এবং brain-এর outer covering dura mater-এর মধ্যবর্তী স্থানে রক্ত জমে যাওয়া।

Clinical ও forensic medicine-এ এই পরিস্থিতিকে কখনও কখনও “talk and die” syndrome বলা হয়—কারণ রোগী প্রথমে স্বাভাবিকভাবে কথা বলতে পারেন, কিন্তু পরবর্তীতে দ্রুত অবনতি হয়ে মৃত্যুঝুঁকি তৈরি হতে পারে।

Classic Three-Phase Timeline

১. Initial Unconsciousness — প্রাথমিক অচেতনতা

একটি blunt force impact—যেমন sports injury, fall বা assault—এর ফলে concussion বা mild traumatic brain injury (TBI) হতে পারে। রোগী সাময়িকভাবে unconscious হয়ে যেতে পারেন অথবা confused/dazed অবস্থায় থাকতে পারেন।

২. Lucid Interval — সাময়িক স্বাভাবিকতার পর্যায়

এরপর রোগী আবার consciousness ফিরে পান। তিনি প্রশ্নের উত্তর দিতে পারেন, হাঁটতে পারেন এবং অনেক সময় সম্পূর্ণ স্বাভাবিক আচরণও করতে পারেন।

এই lucid interval কয়েক মিনিট থেকে কয়েক ঘণ্টা পর্যন্ত স্থায়ী হতে পারে; বিরল ক্ষেত্রে এটি ২৪ ঘণ্টারও বেশি হতে পারে।

৩. Rapid Neurological Deterioration — দ্রুত neurological অবনতি

এরপর হঠাৎ শুরু হতে পারে—

severe headache

vomiting

increasing drowsiness

confusion

focal neurological deficit

loss of consciousness

coma

এবং শেষ পর্যন্ত brain herniation ও মৃত্যু।

-কেন Lucid Interval হয়? — Pathophysiology

Lucid interval মূলত intracranial bleeding এবং brain-এর compensatory mechanisms-এর মধ্যবর্তী একটি সাময়িক পর্যায়ের ফল।

১. Arterial Tear

Classically, temporal skull fracture-এর কারণে underlying middle meningeal artery ছিঁড়ে যেতে পারে।

এর ফলে arterial bleeding শুরু হয়ে epidural hematoma তৈরি হয়।

২. Compensatory Window

Skull একটি rigid structure হওয়ায় intracranial space সীমিত।

শুরুতে intracranial blood volume বাড়লেও brain কিছু compensatory mechanism ব্যবহার করে:

CSF displacement

venous blood displacement

intracranial volume redistribution

এই compensation-এর কারণে প্রথমদিকে ICP (intracranial pressure) খুব বেশি না-ও বাড়তে পারে এবং patient clinically relatively normal থাকতে পারেন।

এটাই lucid interval-এর clinical explanation-এর গুরুত্বপূর্ণ অংশ।

৩. Tipping Point

এক পর্যায়ে এই compensatory mechanisms আর যথেষ্ট থাকে না।

তখন অল্প পরিমাণ additional intracranial volume-ও ICP-এর দ্রুত বৃদ্ধি ঘটাতে পারে।

এর ফলে:

↑ ICP → ↓ cerebral perfusion → cerebral ischemia → brain displacement → brain herniation → rapid loss of consciousness

Expanding hematoma brain tissue-এর ওপর pressure সৃষ্টি করে এবং cerebral blood flow কমিয়ে দিতে পারে। গুরুতর ক্ষেত্রে uncal/ transtentorial herniation হয়ে brainstem compression হতে পারে।

Red Flags — Warning Signs:

Lucid interval চলাকালীন রোগী বাইরে থেকে relatively normal মনে হলেও intracranial hematoma ধীরে ধীরে expand করতে পারে।

নিচের যেকোনো লক্ষণ দেখা দিলে এটি medical emergency:

Progressively worsening severe headache

Nausea and vomiting

Increasing drowsiness

Confusion বা irritability

Anisocoria — দুই চোখের pupil-এর size অসমান হওয়া

এক পাশের শরীরে weakness বা paralysis

পুনরায় loss of consciousness

Seizure

Altered mental status

বিশেষভাবে head injury-এর পর consciousness ফিরে আসার পর পুনরায় neurological deterioration হলে intracranial hematoma অত্যন্ত গুরুত্বের সঙ্গে বিবেচনা করতে হবে।



Clinical Pearl

সব epidural hematoma-তেই lucid interval থাকবে—এমন নয়। আবার lucid interval থাকলেই সেটি EDH—এমনও নয়।

তবে:

Head injury → initial loss/alteration of consciousness → apparent recovery → subsequent rapid neurological deterioration

এই classic sequence দেখা গেলে epidural hematomaসহ intracranial hemorrhage অত্যন্ত জরুরি differential diagnosis।

“Patient looks normal” does NOT mean “brain injury is absent.”

Head injury-এর পর নতুন করে headache, vomiting, drowsiness, confusion, unequal pupils বা weakness দেখা দিলে অবিলম্বে emergency medical evaluation এবং appropriate neuroimaging প্রয়োজন।

Lifestyle intervention vs placebo: lower odds of multimorbidity

OR 0.55 (95% CI 0.35–0.88).

The study followed participants from the original DPP through 2021, approximately 21 years after randomization. Importantly, this was an observational follow-up analysis of the original randomized cohort, rather than a new 21-year randomized trial.

Bottom line: The evidence supports intensive lifestyle intervention as a durable strategy to reduce multimorbidity burden in adults with prediabetes, whereas this particular analysis did not demonstrate a comparable benefit from metformin.

This is an important long-term Diabetes Prevention Program (DPP) finding. In simple clinical terms:

Among adults with prediabetes, intensive lifestyle intervention—primarily weight loss, healthier diet, and increased physical activity—was associated with a lower risk of multimorbidity over 21 years compared with placebo.

Multimorbidity was defined as having ≥2 chronic conditions.

Metformin did not significantly reduce multimorbidity compared with placebo.

The protective association remained even when diabetes itself was excluded from the multimorbidity definition.

Lifestyle intervention also reduced the occurrence of combinations (“dyads”) of high-cost chronic diseases, suggesting potential benefits beyond diabetes prevention alone.

Clinical takeaway

The message is broader than simply “lifestyle prevents diabetes.”

For people with prediabetes, sustained lifestyle modification may help prevent or delay diabetes while also reducing the long-term burden of multiple chronic diseases.

The EAN guideline gives a strong recommendation :
For long-term treatment, carbamazepine or oxcarbazepine are recommended as drugs of first choice. Lamotrigine, gabapentin, botulinum toxin type A, pregabalin, baclofen and phenytoin may be used either alone or as add-on therapy.

> Carbamazepine (CBZ) and oxcarbazepine (OXC) remain the established first-choice pharmacological treatments for trigeminal neuralgia (TN). This position is supported by the American Academy of Neurology (AAN)/European Federation of Neurological Societies (EFNS) evidence-based guideline and the European Academy of Neurology (EAN) guideline. The AAN/EFNS guideline assigns Level A evidence to carbamazepine and Level B evidence to oxcarbazepine, while the EAN guideline gives a strong recommendation for both drugs as first-choice long-term therapy.

Actual scientific evidence based explanation according to
WHO/IARC-এর ২০২৬ সালের নতুন global analysis অনুযায়ী, 2022 সালের নতুন cancer cases-এর 37%—প্রায় 7.1 million cases—preventable causes-এর সঙ্গে attributable ছিল।

এর মধ্যে to***co, alcohol, high BMI, physical inactivity, air pollution, UV radiation এবং cancer-causing infections অন্তর্ভুক্ত।

High-temperature cooking ও Acrylamide

সবচেয়ে গুরুত্বপূর্ণ mechanism:

Ethanol → Acetaldehyde → DNA damage

Acetaldehyde একটি genotoxic metabolite, যা DNA damage ও carcinogenic mutations ঘটাতে পারে। এছাড়াও oxidative stress, folate metabolism alteration, hormonal effects এবং epigenetic changes ভূমিকা রাখতে পারে।

অর্থাৎ oral cancer-এর ক্ষেত্রে alcohol-এর biological plausibility এবং epidemiological evidence—দুটিই শক্তিশালী।

*Pesticides ও “শাকসবজি

কারণ:

Pesticide exposure ≠ pesticide residue থাকা vegetable consumption

Occupational/high-level pesticide exposure এবং dietary exposure এক জিনিস নয়।

আরও গুরুত্বপূর্ণ হলো, সব pesticide carcinogenic নয় এবং সব organophosphate-এর cancer evidence একই নয়। Cancer risk নির্ভর করে:

occupational vs dietary exposure
