Diabetes Doesn’t Just Affect Blood Sugar — It Can Steal Your Vision Silently.

A Real-Life Case That Speaks Volumes:

A 56-year-old patient living with uncontrolled Diabetes Mellitus for 18 years, began experiencing vision loss due to retinal hemorrhage.

Treated with Avastin injections
Followed by Retinal Laser (PRP) therapy
Eventually required Vitrectomy with Silicone Oil Tamponade (SOR)

Later, the patient developed:
🔹 Cataract
🔹 Secondary Glaucoma

Current Visual Acuity:
➤ Right Eye: 6/24
➤ Left Eye: 6/36
(Vision significantly reduced from normal)

Diabetic eye disease can progress silently — and by the time symptoms appear, irreversible damage may have already occurred.

✅ All diabetic patients should have a comprehensive retinal exam every 6 months
✅ Timely anti-VEGF injections, laser therapy, or surgical intervention can prevent blindness.

Some high-quality slit-lamp photos of anterior uveitis (iritis) to help you visualize the condition:

1️⃣ Ciliary injection: A red ring around the cornea, indicating deep inflammation (seen in upper-left of image 1)

2️⃣ Cells & flare in the anterior chamber: Small white specks ("cells") and protein haze ("flare") inside the front chamber—clearly shown in image 2

3️⃣ Keratic precipitates (KPs): Clumps of inflammatory cells on the corneal endothelium—visible in image 2 and image 4

4️⃣ Hypopyon(in severe cases): Layered white eye exudates at the bottom of the anterior chamber—seen in image 1, denoted “C”

5️⃣ Pupil changes: Irregular or constricted pupil due to inflammation and adhesions (posterior synechiae)—noticeable in image 4

◼️ How to Identify Anterior Uveitis

Look for these key clinical clues under a slit-lamp exam:

1. Red eye with ciliary flush (deep redness around the limbus rather than conjunctival redness)
2. Anterior chamber reaction: visible cells and flare
3. Keratic precipitates on the cornea
4. Pupil irregularities from adhesions
5. Hypopyon in more severe or infectious cases

Primary Eye Care & management ( PART lI )

♦️ Dry Eye Syndrome (Keratoconjunctivitis Sicca)

Symptoms: Grittiness, blurred vision, worse with screen time

Signs: Reduced tear break-up time, punctate epithelial erosions

Management:

* Artificial tears (non-preserved preferred)
* Lid hygiene and warm compresses
* Consider omega-3 supplements
* Refer if no response or associated with systemic disease (e.g., Sjögren’s)

♦️ Blepharitis

Symptoms: Itchy, red eyelids with crusting; often chronic

Signs: Lid margin inflammation, dandruff-like scales

Management:

* Warm compresses
* Lid scrubs (baby shampoo or commercial lid wipes)
* Antibiotic ointment (e.g., erythromycin) if secondary infection
* Consider oral doxycycline for chronic cases

♦️ Hordeolum (Stye) / Chalazion

Symptoms: Localized, painful (hordeolum) or painless (chalazion) eyelid swelling

Management:

* Warm compresses several times a day
* Topical antibiotic ointment if draining
* Refer for incision & curettage if persistent >4 weeks

♦️ Subconjunctival Hemorrhage

Symptoms: Sudden red patch in eye, typically painless

Causes: Valsalva, trauma, hypertension, anticoagulants

Management:

* Reassure — usually self-resolves in 1–2 weeks
* Check BP, INR (if on warfarin), and bleeding disorders if recurrent
* Lubricants for comfort

♦️ Diabetic Retinopathy

Symptoms: Often asymptomatic until late; may cause blurry or patchy vision

Signs (on fundus exam): Microaneurysms, hemorrhages, cotton wool spots

Management:

* Yearly dilated fundus exams
* Strict glycemic, BP, and lipid control
* Refer to ophthalmologist for laser or anti-VEGF therapy if needed

♦️ Retinal Detachment

Symptoms: Sudden floaters, flashes, curtain-like shadow over vision

Management:

* Urgent referral to ophthalmology
* Avoid pressure on eye
* Maintain calm, supine position

🔷 🔷 Subconjunctival Hemorrhage🔷🔷

◾ Sign/Symptoms :

-A bright red patch (blood) on the white (sclera) of the eye
-No pain
-No Change in Vision

◾ History :

- Trauma
- Valsalva maneuver

◾ Treatment :

Most subconjunctival hemorrhages resolve on their own within 1-2 weeks as the blood is reabsorbed by the body. The red patch may initially appear bright red but will eventually fade to yellow or brown before disappearing.

-Observation
-Work up if repeated

🔷 🔷 Bacterial Conjunctivitis 🔷🔷

◾ Sign/Symptoms :

- Redness
- Thick purulent discharge

◾Elicit Hx of :

- Stick contact
- STD's

◾Treatment :

- Broad spectrum antibiotics
- Hygiene

🔷 🔷 Viral Conjunctivitis 🔷🔷

◾Sign/Symptoms :

- Redness
- Watery discharge
- No pain
- No change of vision

◾ Elicit Hx of :

- Stick contact
- Recent URI

◾ Treatment :

- Symptomatic relief
- Cool compression
- Lid & hand hygiene

🔷 🔷 Allergic Conjunctivitis 🔷🔷

◾ Sign/ Symptoms :

- Itching
- Redness
- Stringing mucous discharge
- No pain
-Chemosis

◾Elicit Hx of :

- Allergies
- Seasonal timing

◾ Treatment :

- Mast cell stabilizer
- Short course of steroid

🔷🔷 Keratitis🔷🔷

Infectious :

◾Bacterial
◾Fungal
◾Viral (HSV)
◾Acanthamoebal

Non infectious :

◾ Autoimmune disease

Sign/Symptoms :

- Decrease vision
- Pain
- Light sensitivity
- Corneal opacity
- Hypopyon
- Corneal edema

Elicit Hx of :

- Trauma
- Contact lens

Treatment :

- Smear and culture
- Broad spectrum antimicrobials
- Cycloplegics for pain relief

🔷🔷 Uveitis🔷🔷

◾Sign/ Symptoms :

- Decrease vision
- Pain
- Light sensitivity
- keratitic precipitant
- Anterior Chamber cells
- Hypopyon
- Irregular pupil
- Vitreous cells

◾Treatment :

- Focused on treating the systemic conditions, if any.
- Inflammation of the eye will mostly resolve with topical steroids.
- Cycloplegics for pain relief.
- Sometimes, oral steroids are necessary.
- If chronic, then steroid-sparing agents are necessary.

🔷🔷 Endophthalmitis🔷🔷

- Infection of the globe
- Presents with loss of the vision, pain,redness, Hypopyon
- Inflammation throughout the eye
- Can be bacterial, Viral or Fungal

◾ Treatment :

- Medication vs Surgical

🔷🔷 Acute Angle Closure Glaucoma 🔷🔷

◾ Symptoms :

- Pain
- Decrease vision
- Nausea and vomiting

◾ Signs :

- Increased IOP
- Shallow anterior Chamber
- Corneal edema
- Mid-dilated nonreactive pupil

◾Treatment :

- Anti-glaucoma medication
- Laser peripheral iridotomy

🔷🔷 Open Angle Glaucoma 🔷🔷

- Second leading cause of blindness worldwide
- Most common form
- Asymptomatic until late in course when visual field loss is evident.

◾Risk factor :

- Age
- Family
- Myopic
- Diabetes
- Thin cornea

◾ Sign:

- Increased IOP
- Increased Cup/Disc ratio
- Visual field defect
- Nerve fiber loss in retina/disc

◾ Treatment :

- IOP lowering medication
- Laser procedures
- Surgical procedures
