MUBASSHIRA, a nightmare case.
- Age: 6M | Gender: Female | Weight: 4.8 kg | Hb%: 10.8 g/dl
- History: Repeated RTI, H/O anorectal malformation
- Diagnosis: Large PDA- 6 mm, , AS, Mild COA
- Procedure: PDA device closure, Device size: 10/8

Initially 10x8 device selected but it was not crossing the delivery system. We change cable to thick one and failed . PTS haemodynamics became unstable. We decided to use MFO through JR guide catheter. MFO 8x6 appeared small with residual shunt . It caused coarctation also. So we removed this device. Later we changed PDA delivery system of 6f new one. This time we crossed with 10x8 device with difficulty. Ultimately were able to do it with10x8 ADO. We pressed the device with 8x3 tyshak balloon and gave the aortic disc concave shape. No coarctation was noticed at the end . Procedure took 90 mins.

1. TAJIM

- History: Repeated RTI, Reluctant to feed, Failure to thrive.
- Diagnosis: large PDA- 4 mm
- Procedure: PDA device closure, Device size: 8/6 cocoon

2. MORIUM
- Age: 11 M | Gender: Female | Weight: 8.2 kg | Hb%: 9.9 g/dl
- History: Repeated RTI, Reluctant to feed, Failure to thrive.
- Diagnosis: Large PDA- 5.5 mm,MOD COA,
- Procedure: PDA device closure, Device size: 8/6 cocoon. Balloon coarctoplasty postponed as there was no gradient across descending aorta

- History: H/O bluish coloration of lips & fingers at neonatal stage, Repeated RTI, Mild cough for 3 days.
- Diagnosis: Coronary cameral Fistula
- Procedure: Fistula device closure, Device size:10/8
Aortography showed large long Fistula from LCA to RV with two narrow points in the course.
LCA engaged with left coronery catheter. Wire crossed to RV and exchanged with JR guide catheter of 6 french . 10x6 MFO device loaded to loader and forwarded to Fistula. First disc released to distal part after first constriction, later rest. Part of the device released and positioned . Coronaries are checked frequently to see whether they are free from obstruction by device. Last aortography showed good LCA branches fillings and no residual shunt through Fistula.

One case of PDA stenting in 17 days old baby , one VSD device closure in Fenestrated VSD, four PDA device closure and two ASD device closure were performed today. Seven cases were performed in 3 hours and PDA stenting in 75 minutes .
