Day 7: TERATOGENICITY

Answer: D — Methotrexate
Methotrexate is an absolute contraindication in pregnancy — it is a folic acid antagonist that inhibits DNA synthesis, causing severe fetal malformations and fetal death. It is used to treat ectopic pregnancies precisely because of this effect. Patients must use reliable contraception during and for at least 3 months after stopping methotrexate. Azithromycin and heparin are considered compatible with pregnancy. Acetaminophen at recommended doses is generally safe. Other teratogens to know: isotretinoin, valproic acid (neural tube defects), warfarin (weeks 6–12), and ACE inhibitors/ARBs in the 2nd and 3rd trimester.
