The Upper Canada Family Health Team is comprised of a group of health care professionals working collaboratively to provide comprehensive and accessible health care. The goals of the Upper Canada Family Health Team include preventing illness and promoting the health of patients. We aim to reduce the number of patients in Brockville who do not have a health care provider, reduce ER visits for minor illness, and reduce hospital admissions for chronic illnesses.

Please note that Upper Canada Family Health Team sites and the After-Hours Clinic will be closed on Monday, August 3, 2026, in recognition of the Civic Holiday.

We will reopen on Tuesday, August 4, 2026, and resume regular office hours.
