RE: Closure of medical practice

After months of deliberation, I have made the difficult decision to close my private family practice clinic as of September 30, 2022. I will continue to monitor incoming diagnostics and follow up where appropriate until October 31, 2022, at which point patients will be expected to review any new medical reports with another primary care physician.

I will continue to work at the Burin Peninsula Health Care Center providing services to our area as well as take this time to explore other career goals.

Unfortunately, due to current circumstances in Newfoundland and Labrador there is no physician taking over my practice. I encourage all patients to try to seek a new physician to the best of their abilities and if they are unsuccessful to place their name on the list for a future collaborative team clinic through patient connect at 1.833.913.4679 or email patientconnect.nlchi.nl.ca.

While I recognize that this will be very stressful for patients, I will do my best to support you prior to my clinic closure. With an appointment I can refill regular medications for up to 1 year to allow more time to find a new physician.

I have been in contact with a company regarding storage of medical records and more details of this will be provided however at this time if you would like a copy of your medical record please contact my medical office administrator, Kayla and we will arrange for this to be available.

Urgent: COVID 19 Clinic Update

Due to the ongoing 4th wave of the highly contagious omicron variant, starting immediately our clinics will be switching to predominantly virtual care appointments where possible.

Our line of work (especially at the hospital) causes us to be at a much higher risk of exposure and therefore we need to limit our interactions where possible. This is to ultimately protect our patients.

In person appointments will be on a case by case bases as reviewed by your doctor. Kayla will call those who need to be seen in person. Please note that those needing to come into the clinic may have to have their appointments time/date changed to allow for the safe spacing of patients and cleaning protocols. Furthermore those coming in person will need call the clinic to answer the COVID 19 screening questions prior to entering. Anyone with any of the following symptoms (new or worsening) should not come into the clinic:

- Fever or chills
- Cough
- Shortness of breath or difficulty breathing
- Fatigue
- Muscle or body aches
- Headache
- New loss of taste or smell
- Sore throat
- Congestion or runny nose
- Nausea or vomiting
- Diarrhea

Also please do not enter the clinic if you are in isolation pending a swab result, regardless of your symptoms.

Anyone feeling severely unwell should proceed to their nearest ER as they regularly would. We also strongly encourage anyone experiencing any symptoms of COVID 19 to arrange for testing.

We thank you so much for your understanding and patience in these unprecedented times. We will be reviewing this policy change on a weekly bases and look forward to going back to our previous policies when it is safe to do so. Hold fast and stay safe.

Due to unforeseen circumstances the front end of the clinic will be closed for today and friday. No phone calls will be answered during this time. *** ATTENTION REGARDING WAIT TIMES***

Currently my wait time for next available appointment is approximately 4-6 weeks away as some slots are kept for urgent recalls of diagnostic testing. Other physicians in this area also have similar wait times. This unfortunately is not just due to the Covid-19 pandemic as wait times were prolonged since opening my practice in 2018. Upon opening I rostered too many patients in an effort to help as many people as possible not knowing the complex care and chronic care needs of our population on the Burin Peninsula. To provide optimum care to my patients many of them require a high volume of visits to manage their medical conditions.

Other reasons for long wait times include:
- I provide ER coverage as well as OR assist at the Burin Peninsula Health Care Centre
- When I take any days off, I am unable to get locum coverage in Burin which further backlogs the clinic
- I must ensure that I take care of myself with boundaries in place to limit work hours and after-hours charting, paperwork, insurance forms etc. I have experienced significant burnout in the early stages of my career and had plans to leave the area due to the demands on family physicians in Burin and our lack of support from our government. I need to take care of myself in order to stay in Burin and take care of you!

While I understand the frustrations of wait times, my wonderful MOA, Kayla nor myself can make appointments where none exist. I would like to remind everyone that kindness matters, and rude or disrespectful behavior will not be tolerated as explicitly stated in your patient contract.

I strongly encourage every patient to contact their MHA. We are in the middle of a family medicine crisis in our province that our government refuses to acknowledge. Most of my friends that I trained with have left the province or do not practice clinic-based family medicine due to a system that fails both physicians and patients. Make your voices heard that we all need access to better primary care in Newfoundland and Labrador.

** Summer Holidays Reminder **

We have now started our summer holidays. Dr. Slaney will be away from the office and won’t be seeing patients in her clinic from Aug 13-20th inclusive. If you have a scheduled virtual appointment on the 11th or 12th of August this will be going ahead. This applies to Dr. Slaney’s patients only and is for summer holidays and covering services at the BPHCC. Please note the clinic is closed Aug 9-21 inclusive for Kayla’s holidays. During the time Kayla is away no phones or faxes will be answered. If you have any issues please see your pharmacy team or proceed to your nearest ER if it’s an emergency.

Medication refills:
As of June 1st I have stopped providing refills for medications outside of appointments. The volume of requests is unreasonable and takes a significant amount of time as well medications should be reviewed in an appointment prior to a refill. Patients have been advised to schedule their 3 or 6 month refill appointments in advance and failure to do so may result in having to pay for medication management at your pharmacy or attending the ER for a refill.

Clinic behavior policy:
This is a reminder to patients that when they applied to join my practice a policy was signed which detailed expectations around patient behavior. Rude or disrespectful behavior in the office, on the telephone or on social media will not be tolerated at Dr. P. Slaney Family practice and this includes Dr. Slaney and her staff. Everyone deserves to work in a safe and respectful environment and any behavior that goes against this policy will be grounds for terminating the patient-physician relationship.

Increased availability of in-office appointments:
As of September 1st, 2021 when you call to schedule an appointment you will be given the option of in-office or telephone appointment as at that time most of my vulnerable population will have been fully vaccinated and it will be much safer to have an increased volume of patients in the office. This is contingent on no further outbreaks or changes in our public health level (currently at level 2) as well as the government continuing to be supportive of virtual care.

Clinic appointment times:
Each clinic appointment, both in office and telephone, is scheduled for 15 minutes. While I try my best to assess multiple problems this may not always be possible. This 15-minute time slot includes time to document the encounter in the chart, order investigations as needed and send consults. I will advise patients if we are running out of time and appreciate your cooperation.

Missed clinic appointments:
As per our initial office policy missed appointments will result in a $20.00 no-show fee which must be paid prior to your next appointment. If a patient has three no-shows, this is grounds for discharge from clinic. There are many patients requesting appointments and no-shows prolongs wait-times for everyone. This policy also applies to virtual appointments.

Out of office notices:
Dr. Slaney will be unavailable from June 25th to July 2nd inclusive however the office will be open and you can call for appointments.
Dr. P. Slaney Family Practice will be closed for summer vacation including office staff from August 9th to 20th inclusive. There will be very limited availability of appointments in August due to vacation and ER/OR commitments so please plan ahead for your health care needs.

Our clinic had a delayed opening this morning due to an issue with the Burin Pharmacy. This IS NOT Covid-19 related. We are now open however there is no access to the pharmacy. I have recently been advised via a memo regarding an update for blood collection at the Burin Peninsula Health Care Center. Due to the most recent level 5 shut down the lab is experiencing a back log as well as high call volumes. Patients are now being asked to call 891 3307 to schedule their non-urgent blood work appointment. Please do not go to the hospital without an appointment. We appreciate your cooperation and patience at this time.

Clinic is going ahead today February 1, 2021 however our telephones are not working after Eastlink performed some local maintenance work on Friday. We have been advised they will be at our office as quickly as possible to fix this issue. Important reminder:

I have recently been receiving an increased volume of prescription renewal requests. Please schedule your 3 month follow-up appointments after your visit to ensure you do not run out. While the majority of visits are still virtual at this time I am effectively able to discuss your medications and renew them as appropriate in a virtual appointment. If you allow your medications to run out you may have to pay for medication management at your pharmacy during which they will provide tie-over medications until you have an appointment. I am unable to provide repeated 3 month refills for patients who do not have follow-up appointments as this can lead to missed diagnoses and poor patient-centered care. I appreciate your cooperation. Dr. Slaney will be holding Flu Shot Clinics for Established Patients ONLY…
These clinics will be BY APPOINTMENT ONLY.
Clinic dates and time include:
November 4 11:00 am – 4:00 pm
November 9 10:00 am – 4:00 pm

More clinics may be added based on need.

Patients will be seen by appointment only. You may call the clinic at 342 2285 to schedule an appointment.

Due to the current pandemic, there will be policies in place that have to be followed.
Doors will not be open until the start time. DO NOT enter the clinic until your scheduled appointment time. There will be a consent form to sign prior to receiving your flu vaccine. Patients will enter through the front door at their appointment time and exit through our back door once vaccine has been administered. MASKS ARE MANDATORY!!
Once you have left the building, you will be required to wait in your vehicle for at least 15 minutes to monitor for signs of an allergic reaction. If you should have any reaction to the vaccines you must re-enter the clinic immediately to be assisted.

DO NOT ENTER THE OFFICE IF YOU HAVE ANY FLU LIKE SYMPTOMS OR IF YOU HAVE TRAVELLED OR HAVE BEEN IN CONTACT WITH ANYONE THAT TRAVELLED WITHIN THE 14 DAYS PRIOR TO YOUR APPOINTMENT INCLUDING TRAVEL TO AND FROM HOT ZONES IN NEW BRUNSWICK.

Updates regarding Dr. Slaney's practice:

Our office is open and always has been. We have been working very hard throughout this entire pandemic. To provide timely and safe care Dr. Slaney continues to be available for virtual appointments, during these appointments if Dr. Slaney feels you require an in-office assessment she will make these arrangements or advise her office assistant to contact you. We are unable to provide our usual clinic appointments due to having to follow the current public health regulations. It is important for patients to be aware that getting patients in the office, cleaning the exam rooms and preventing congestion in the waiting room takes a significant amount of time as well we must be mindful of our PPE usage. While we would all like to return to a normal clinic routine this is not possible at this time and we appreciate your cooperation.

If you have a health concern please call the office and Kayla will provide you with the next available virtual appointment and Dr. Slaney will contact you via telephone during that time.

Our office will be closed for summer holidays from ***July 27th to August 9th***. Please ensure that you have any prescriptions that you require prior to the office closure.
