UNA Local 115 serves the Foothills Medical Centre (FMC), who operates in partnership with UofC (Faculty of Medicine), Grace Women’s Health Centre, NW Hemodialysis, South Calgary Hemodialysis / Urgent Care / Mental Health and Fanning Hemodialysis. The Foothills Hospital operates in partnership with several local medical facilities including the University of Calgary (Faculty of Medicine), the Tom Baker Cancer Centre, Grace Women’s Health Centre, South Calgary Dialysis / Urgent Care / Mental Health and Fanning Dialysis. Office Hours: Mon-Fri 8:30 am - 4:30 pm

This is the Canada Post recommended date to mail back special ballot packages to guarantee delivery by receiving deadline (within Alberta). Ensure your full name, current address, and photo are visible on your photocopied, govt issued photo ID! Your outer envelope (envelope B) must include:
Your signed special ballot declaration, copy of your authorized identification, and
Envelope A with the ballots sealed inside.
